뇌졸중환자의 손 기능 회복에 인지운동치료의 효과

국문 초록

목 적 감각장애를 주로 호소하는 뇌졸중 환자를 대상으로 적용한 인지운동치료가 손 기능회복에 미치는 효과를 알아보고자 하였다. 방 법 본 연구의 대상자는 자발성 뇌실질 내출혈(우측시상)로 인해 좌측 편마비가 된 55세 남자 환자로 2009년 6월 1일부터 8월 18일까지 8주 동안 하루 1회 30분씩 팔에 대한 인지운동치료를 적용하였다. 연구절차는 치료 시점, 치료 시작 4주 후, 연구 종료 시기인 치료 시작 8주 후에 동일한 방법으로 객관적 평가와 인지운동치료 평가를 이용하여 전후 평가 결과를 비교하였다. 결 과 팔에 대한 인지운동치료 후 일상생활동작(MBI)과 감각인식 및 마비측 손 기능이 향상되었다. 또한 인지운동치료 프로파일에서 병리적 특성, 인식 양상, 주의집중 양상, 이미지 양상, 언어 양상, 학양상에서도 개선을 보였다. 결 론 뇌졸중으로 인한 편마비 환자의 팔 기능회복 을 위한 치료적 접근 중 인지운동치료가 손 기능회복에 긍정적인 효과가 있었다.

영문 초록

Objective : The study aimed to look into the effects of the cognitive therapeutic exercise for recovering the upper extremity function of the patient with hemiplegia, on the recovery of hand function, targeting the patient complaining of the sensory impairment. Methods : The subject of the study was male patient with left hemiplegia caused by autonomous parenchymal hematoma(right thalamus) who aged 55, and the cognitive therapeutic exercise was applied to him for 30 minutes once a day for 8 weeks from June 1, 2009 to August 18, 2009. Regarding the procedures of the study, pre and post-test results in the treatment time, 4 weeks after starting treatment, and 8 weeks after starting treatment which is the end time of the study with the same methods were compared by using the objective test and the cognitive therapeutic exercise test. Results : After the cognitive therapeutic exercise regarding upper extremity, Activities of Daily Living(MBI), Sensory Perception and hand function if affected side are increased. The cognitive therapeutic exercise profile showed the improvement in pathological characteristics, especially in recognition aspect, attention aspect, image aspect, language aspect, and learning aspect. Conclusion : There was the significant effect of the cognitive therapeutic exercise, among therapeutic approaches for recovering the upper extrimity function of the patient with hemiplegia, on the recovery of hand function.
