급성 뇌졸중 환자에서 손상측 하지의 인식양상 증진이 밀기증후 감소에 미치는 영향

국문 초록

목 적 밀기증후군을 나타내는 편마비 환자에서 인지운동치료를 사용한 인식 양상 증진이밀기증 후 치료에 미치는 영향에 대한 기초 자료를 제시하고자 한다. 방 법 본 연구의 대상자는 뇌경색으로 인해 좌측 편마비를 가진 69세 남자 환자로 2009년 9월 1일 부터 9월 22일까지 3주 동안 하루 1회 45분씩 인지운동치료를 적용하였다. 인지운동치료는 손상측 하지에 대한 인식양상 개선을 위하여 스펀지를 사용한 압각훈련, 무게추를 사용한 무게감각 훈련, 패그보드를 사용한 거리감각 훈련을 각각 수행하였다. 결 과 초기평가 3주 후에 재평가 실시 결과 MBI를 통한 일상생활동작 평가 시에 49점에서 63점으로 향상되었으며, MMAS를 사용한 운동기능 평가에서 balance sitting항목과 sitting to standing항이각각 2점, 1점에서 5점, 5점으로 향상되었고, SCP를 통한 밀기증후 평가 시에 4.75점에서 1점으로 밀기증후의 변화를 관찰했다. 결 론 스펀지, 무게추, 패그보드를 이용한 인지운동치료에서 인식양상 증진은 밀기증후를 보이는 급성 뇌졸중 환자의 치료에 효과가 있었다.

영문 초록

Objective : The study was designed to provide basic data on how growing recognition pattern using cognitive therapeutic exercise affects the treatment of pusher syndrome in hemiparalysis patients. Methods : The subject of this study was a male patient at 69 of age who suffered from cerebral infarction- induced hemiparalysis of the left side of the body and for whom a 45-minute session of cognitive therapeutic exercise was administered every day for three weeks from September 1st to 22nd, 2009. The cognitive therapeutic exercise consisted of pressure sense exercise using sponge, weight movement exercise using balance weight, and distance sense exercise using peg boards, with the aim of improving perception patterns for impaired lower limbs. Results : It was found in a test three weeks of the initial test that the score on a daily movement test via MBI rose to 63 points from 49, on motor function test using MMAS, scores of both balance sitting and sitting to standing grew to 5 and 5, respectively, from 2 and 1, and on a pusher syndrome test using SCP, pusher syndrome value changed from 1 to 4.75. Conclusion : The study showed that recognition patterns enhanced through cognitive therapeutic exercise using sponge, peg board, and short-axis platform had an influence on the treatment of acute stroke patients who had pusher syndrome.
