뇌졸중 환자의 상지 신장반사 제어가 손 기능 회복에 미치는 효과

국문 초록

목 적 뇌졸중 환자에게 인지운동치료로 접근했을 때 상지의 신장반사 제어가 손의 기능회복에 효과가 있는지를 알아보고자 하였다. 방 법 본 연구의 대상자는 56세 남성으로 두개 내 뇌출혈로 인해 우측 편마비 환자로 2009년 7월 17일부터 2009년 10월 10일까지 하루 30분씩 신장반사 제어를 위한 인지운동치료를 적용하였다. 연구절차는 일상생활동작영역(FIM)과 상지운동기능영역(MFT)과 인지기능평가(MMSE-K, LOTCA)와 인지운동치료 프로파일을 초기평가와 재평가를 하여 전후 비교 분석 하였다. 결 과 인지운동치료 후에 일상생활동작영역은 초기 평가 시 104점, 후기 평가에서 107점으로 3점 향상보였고, 상지운동기능영역 평가는 환측 상지가 초기평가보다 재평가 시 5점 상승 보였다. 인지기능평가영역(MMSE-K) 초기평가 13점에서 후기평가에 18점으로 3점 향상하였다. 인지기능평가영역(LOTCA)에서 운동실행 영역에서 모지대립이 가능하게 되고 물건들의 사용 능력이 향상 되었다. 인지운동치료 프로파일에서는 병리적 특성 중 특히 신장반사와 방산반응에서 개선을 보였고, 인식양상, 주의집중 양상, 이미지 양상에서 개선을 보였다. 결 론 상지의 신장반사가 나타나는 환자에게 인지운동치료적으로 접근하여 상지의 신장반사를 제어함으로써 손의 기능회 복에 효과가 있음을 알 수 있다.

영문 초록

Objective : For the stroke patients with cognitive therapeutic exercise, the study aims to look into the effects of stretch reflex control of upper extrimity on functional recovery of their hands. Methods : The subject of the study was the male patient with right hemiplegia caused by intracranial hemor rhage who aged 56, and the cognitive therapeutic exercise was applied to him for 30 minutes once a day from July 17, 2009 to October 10, 2009. The procedures of the study were compared and analyzed before and after testing initially and retesting FIM, MFT, MMSE-K, LOTCA, and cognitive therapeutic exercise profiles. Results : After the cognitive therapeutic exercise, FIM showed 104 scores in the initial test and 107 scores, the increase of 3 scores in the late test. MFT test showed the increase of 5 scores regarding the upper extrimity of the affected side in the retest. The initial test of MMSE-K increased to 18 by 3 in the late test. Opposition in running exercise areas of LOTCA becomes possible and object serviceability is improved. The cognitive therapeutic exercise profile showed the improvement in pathological characteristics, especially in stretch reflex and irradiation as well as in recognition aspect, attention aspect, and image aspect. Conclusion : The result of the study found out that it is effective to recover the function of hands by controlling stretch reflex after giving the cognitive therapeutic exercise to the patient showing stretch reflex of upper extremity.
