고관절 수술 후 측방향 검사 시 서혜부 측방향 검사의 유용성 평가

국문 초록

목 적：고관절 수술 후 시행되는 세 가지 X-선 측방향 검사의 영상을 비교 분석하여 효과적인 검사법을 적용하는데 도움을 주고자 하였다. 대상 및 방법： 2006년 10월부터 2017년 12월까지 본원 정형외과에 내원하여 고관절 수술을 시행한 후 단순 X-선 검사를 시행한 200명의 영상을 비교 분석하였다. 고관절 측방향 검사가운데 고관절 횡측방향 검사, 고관절 횡측방향 변형검사, 서혜부 측방향 검사 영상을 대퇴 골두의 왜곡, 소전자의 왜곡 정도, 대퇴골과 관골구 임플란트의 해리 정도, 검사 반대 측 연부조직의 음영 정도, 고관절의 관찰로 구분하여 비교하고 평가하였다. 결 과：대퇴 골두와 소전자의 왜곡정도에서 고관절 횡측방향, 고관절 횡측방향 변형, 서혜부 측방향 검사 사이에 유의한 차이를 보이지 않았다(p＞0.05). 임플란트의 해리 가운데 대퇴골은 4.86±0.43점, 관골구는 4.96±0.23점으로 서혜부 측방향 검사에서 가장 잘 관찰이 되었다(p<0.05). 검사 반대 측 연부 조직은 4.75±0.65점으로 서혜부 측방향 검사에서 가장 영향이 적었으며, 고관절의 관찰도 서혜부 측방향 검사에서 3.74±0.93점으로 가장 우수하게 평가 되었다(p<0.05). 결 론：수술 후 고관절의 측방향 검사 시 서혜부 측방향 검사를 적용함으로써 안정된 자세로 검사 목적에 맞는 유용한 영상을 얻을 수 있을 것으로 사료된다.

영문 초록

Purpose：To compare and analyze radiographs of three x-ray lateral examinations conducted after hip surgery in order to support the application of an effective examination method. Materials and Methods：X-ray images of 200 patients who received simple x-ray examination af-ter hip surgery at our orthopedic department from October 2006 to December 2017 were analyzed. Hip trans-lateral, modified hip trans-lateral, groin lateral examination images from the hip joint lateral examination and distortion of femur head, distortion degree of lesser trochanter, degree of implant loosening of femur and acetabulum, shadow effect of soft tissues was less on the opposite side of the examination, and observation of hip joint were compared and analyzed. Result：The femur head and distortion of lesser tro-chanter showed no significant difference from the hip trans-lateral, modified hip trans-lateral, and groin lateral examinations (p＞0.05). Among implant loosening, femur received a score of 4.86±0.43 and acetabulum received a score of 4.96±0.23, which were best observed during the groin lateral examination (p<0.05). The shadow of soft tissues on the opposite side of the examination received a score of 4.75±0.65, which was the least affected by the groin lateral examination, and the observation of hip joint was also best evaluated with a score of 3.74±0.93(p<0.05). Conclusion：By applying the groin lateral examination during the lateral examination of the hip joint after surgery, useful images can be obtained for the purpose of the examination in a stable position.
