일본 치매노인 지원의 현황과 과제

국문 초록

본 연구는 초고령사회 일본에서 끊임없이 증가하고 있는 치매노인 지원 현황을 살펴보고 이와 관련된 지역포괄케어시스템의 구체적인 사례를 중심으로 그 현황과 과제를 정리하는 것을 목적으로 한다. 2015년 4월부터 개정된 개호보험제도는 지역지원사업의 포괄적 지원사업의 일환으로 ‘치매시책 추진’을 포함하고 있으며, 이는 2012년 9월에 책정된 ‘치매 시책 추진 5개년 계획 (오렌지 플랜)’을 개정하여 2015년 1월 27일에 발표된 ‘치매시책 추진 종합전략; 치매 노인 등에게 친절한 마을 만들기를 위하여’(신 오렌지 플랜)과도 연동된다. 신 오렌지 플랜에서는 ‘조기 발견, 사전대응’을 기본으로 2018년까지 전국에 1)치매 초기 집중 지원팀과 2)치매 지역 지원 추진원 설치를 목표로 한다. 지역포괄케어시스템은 각 지역의 실태에 따라 그 형태와 역할의 다양성이 인정되고 있는 한편, 모든 시스템은 ‘자조(自助)·호조(互助)·공조(共助)·공조(公助)’의 4가지를 전재개념으로 하고 있다. 본 연구에서는 지역포괄케어시스템과 관련한 1)치매 노인 조기발견 및 돌봄 사업(岐阜県 東白川村)과 2)치매에 걸려도 안심하고 살 수 있는 시민협동 네트워크 만들기(福岡県　大牟田市)의 2곳의 사례를 검토하였다. 그 결과, 향후, 치매노인 지원과 관련하여 최소한 필요한 ‘공조(共助)·공조(公助)’ 내용을 밝혀 행정기관의 역할을 제시할 필요가 있음이 시사되었다. 또한, 과연 어디까지 ‘자조(自助)’와 ‘호조(互助)’로 보완이 가능할지에 대한 검토가 필요하며, 동시에 이때에 필요한 공(公)의 역할을 분명하게 할 필요가 있다.

영문 초록

This paper examines the present status of the support for people with senile dementia, which continues to increase in Japan’s super-aging society, and describes the present state of and issues related to the integrated community care systems, illustrating the current situation with concrete examples. The long-term care insurance system, as amended in April 2015, includes “dementia policy promotion” as a local support services component within the comprehensive support services. This new item amended the promotion of the five-year dementia policy “Orange Plan,” which was created in September 2012 and was combined with the “New Orange Plan” announced on January 27, 2015. The New Orange Plan aims to (1) establish early-stage dementia support teams and (2) promote the staff of regional dementia support systems by 2018 for the purpose of early detection and support. Within the integrated community care system, the diversity of the systems and roles in each region of Japan is acknowledged. Simultaneously, self-help, mutual aid, cooperation, and helpful assistance are essential components of all support systems. In the present study, two examples of the integrated community care systems were investigated: (1) a system in which senile dementia can be discovered early so that care services can be provided (Gifu); and (2) a system in which citizen cooperation networks in which people can continue to live safely even if they develop dementia are created (Fukuoka). The results of this study demonstrate the essential services that are necessary to realize effective “cooperation” and to provide “helpful assistance” relating to senile dementia support, as well as the necessary roles for administrative bodies. Additionally, it is necessary to determine the extent to which this support can be complemented by self-help and mutual aid as well as to clarify the roles that the general public can play.
