국문 초록

국가와 관계없이 장수와 관련된 연구는 성공적 노화로서 장수지표의 사회경제적 건강요인에 중점을 두어야 한다는 것은 매우 중요하다. 장수지표에 기여할 수 있는 사회경제적인 건강요인의 역학적 연구는 성공적 노화로서 연관성 요인들을 규명하는데 도움이 될 것이다. 그러므로 본 연구의 목적은 백세인의 장수지표 (LI)을 평가하고 경제개발협력기구(OECD)의 국가들을 중심 으로 백세인의 장수지표에 관한 성공적 노화로서 사회 경제적 건강요인에 대한 상관관계와 영향력을 규명하고자 한다. 사회 경제적 건강 요인의 자료는 OECD통계자료(2005)와 한국 통계청 자료를 활용했으며, 백세인의 인구 통계 자료는 조사자료가 충분한 17 OECD 국가의 통계청에서 발행한 인구 센서스 통계자료(2005) 및 국가별 인터넷 자료를 조사하여 분석하였다. 백세인의 장수지표는 조사대상 국가의 백세인 인구의 데이터베이스를 활용하여 65세 이상 중에서 백세인의 비율을 분석하여 활용하였다. 성공적 노화는 사회 경제적인 건강요인의 지표를 성공적 노화의 개념을 정의하여 활용하였다. 연구결과, 조사대상 OECD국가의 백세인 장수지표는 프랑스(21), 미국, 호주, 캐나다, 뉴질랜드, 영국, 일본 등이 10명 이상이지만 한국이 가장 낮은 2명이었다. 이들 국가의 백세인의 장수지표는 국민총생산(GDP)에서 보건의료비 지출의 비중, 공공부문의 의료비 지출, 65세 이상 및 14세 이하의 부양지수 등의 사회 경제적인 요인은 정(+)의 상관관계를 가지고 있으나, 성인 남성의 흡연비율, 남녀 자살비율, 미세먼지 등은 부(-)의 상관관계를 갖는 것으로 설명력은 85%로 분석되었다 (R-Sq=0.857, P=0.001). 따라서 국가의 고령사회의 역할은 고령인의 복지를 향상시키는 것이 중요하다. 특히, 국민보건예방사업을 강화하고, 고령인의 건강복지 지원사업을 확대할 수 있는 보건의료부문의 재정을 확충해야 한다고 본다.

영문 초록

Background: Regardless of country, it is important that studies concerning longevity should focus on the socio-economic health factors of the longevity index as successful ageing is typically associated with such factors. Therefore, this epidemiological study of the socio-economic health factors that contribute to the longevity index may help to identify the factors associated with successful ageing. Objective: This study estimates the longevity index (LI) of centenarians and elucidates the influence of correlates of socio-economic health factors as successful ageing on the LI of centenarians in countries of the Organisation for Economic Co-operation and Development (OECD). Methods: Social indicators for this study were obtained from 17 OECD countries were included: these were taken from Stat Extracts in OECD and Korean Statistical Information Service. The data for the analysis of centenarians in the selected OECD countries were obtained from the population censuses conducted by the National Statistical Offices of these member countries. This study utilized the demographic databases of centenarians from the above countries to calculate the following parameters: the proportion between the centenarians and the total population above the age of 65 years. For this study, successful aging was defined based on hypothesis of socio-economic health factors as successful ageing. Results: Although significant positive correlations were found between the LI of centenarians and socio-economic health factors such as health expenditure as a percentage of the gross domestic product, and public expenditure on health and dependency ratio, negative correlations were found between the LI of centenarians and social factors such as, suicide, smoking and environmental particulate matter (R-Sq=85.7, P=0.001). Conclusions: The findings imply that an aged society plays an important role in ensuring the continued welfare of centenarians; therefore, the governments of OECD countries must strengthen their preventive health care programmers and increase their existing support services for the elderly through increased financing of the health sector.
