건강보험료 부과대상 실질 소득 파악: 서베이 및 공부상 자료의 결합․비교 분석

국문 초록

우리나라 국민건강보험의 직장 및 지역으로 이원화된 건강보험료 부과체계는 건강보험료 부담 형평성 논란을 야기해 왔다. 현재 소득 기반의 일원화된 건강보험료 부과 체계로 개편되어야 하는 것에는 사회적으로 합의가 되어 있지만 이를 위해서는 직장가입자와 지역가입자의 실제 소득 파악률에 있어서 큰 차이가 없어야 한다. 본 연구는 실제 소득 파악률을 알기 위하여 서베이 자료인 의료패널자료에 본인이 응답한 소득과 국세청의 공부상 자료 에서 파악하고 있는 소득을 개인단위 및 가구단위로 결합하여 분석하였다. 직장가입자의 소득 파악률은 근로소득자 개인의 경우 100%에 근접하였고 가구의 경우에도 약 80%로 나타났 다. 반면 지역가입자의 소득 파악률은 개인 단위와 세대 단위의 구분 없이 20%~30%로 저조 하였다. 따라서 소득만을 기반으로 하는 건강보험료 부과체계 대신에 기본적으로 소득에 바탕을 두고 보험료를 부과하지만 소비지출에 대한 보험료 부과를 통해 보완하는 체계가 현실 적으로 적합하다.

영문 초록

It is problematic that employees and the self-employed bear contributions in a different way in the unified Korea National Health Insurance Program. The different contribution system have caused the insured to raise issue of equity. To improve the equity, a unified contribution rate system which should be based on household’s aggregate income is suggested. It is necessary for authorities to have exact household income for two groups in order to implement the unified system based on income. This study investigates the degree of accuracy of income estimates for each group after merging a survey with an administrative dataset the National Health Insurance Service have used for calculating premiums. We find that authorities keep full information of employees’ incomes while they know of only 25% of the self-employed’s true incomes at the individual level. At the household level, authorities know of about 80% of employee’s true incomes while they know of only 25% of the self-employed’s ones. Considering that authorities could not fully collect self-employed’s true incomes, a unified contribution rate system should be based on household’s expenditure as well as its aggregate income.
