콘택트렌즈 착용 불편감을 가지는 성인에서 각막굴절교정렌즈 착용의 순응도 및 안구표면의 변화

국문 초록

목적: 콘택트렌즈 착용 불편감이 있는 정상 근시안 성인에서 각막굴절교정렌즈 착용의 순응도 및 안구표면에 미치는 영향을 알아보았다. 대상과 방법: 가톨릭대학교 성모병원 렌즈클리닉에서 낮 시간에 착용하는 일반 콘택트렌즈 착용 불편감을 호소하는 성인 14명, 28안을 대상으로 수면 시 역기하 각막굴절교정렌즈(overnight orthokeratology, OOK)를 착용시켰다. 설문지 조사를 통한 OOK 렌즈 착용의 순응도 검사와 렌즈 착용 전과 6주 후 구면 대응치, 각막곡률, 중심각막두께를 측정하였다. 안구표면상태 지표에 대한 검사로는 안구 표면질환지수(ocular surface disease index), 눈물 삼투압농도, 쉬르머검사, 눈물막 파괴시간(tear film break-up time), 안구표면 염색점수(ocular surface staining score)를, 마이봄샘 평가로는 눈꺼풀테 이상과 마이봄점수(Meiboscore)로 비교 분석하였다. 결과: 평균 나이는 26.67 ± 3.26세였고, 이전 콘택트렌즈 평균 착용 기간은 2.64 ± 2.41년이었다. 모든 대상자에서 낮 시간에 착용하였던 기존 렌즈에 비하여 OOK 렌즈가 더 편하다는 설문 결과를 보였고, 렌즈 착용 후 굴절 변화 검사인 평균 구면 대응치, 각막곡률 최소값, 중심각막두께 모두 유의한 변화를 보였다(all p<0.001). 안구표면상태 지표와 마이봄샘 평가에서는 OOK 렌즈 착용 1주와 6주 후 착용 전과 비교하여 유의하게 악화된 소견은 없었다(all p>0.05). 결론: 낮 시간에 착용하는 일반 콘택트렌즈 착용 시 불편감이 있어 순응도가 떨어지는 정상 근시안 성인에서 OOK 렌즈는 비교적 안전하고 순응도가 높은 대안이 될 수 있겠다. <대한안과학회지 2016;57(10):1549-1554>

영문 초록

Purpose: To evaluate the effects on the ocular surface and the compliance of overnight orthokeratology lenses wear in normal myopic adults experiencing contact lens discomfort. Methods: In this study, 28 eyes of 14 subjects were analyzed using overnight orthokeratology (OOK). This prospective study investigated the compliance of OOK wear using questionnaires, spherical equivalents, keratometry, and central corneal thickness. Ocular surface disease index, tear osmolarity, Schirmer’s test I, tear film break-up time, and ocular surface staining score were evaluated as ocular surface parameters. Meibomian gland function was evaluated by assessing lid margin abnormality and meiboscore using noncontact meibography. The tests were performed before and 1 and 6 weeks after OOK wear. Results: The mean age of the subjects was 26.67 ± 3.26 years, and the average duration of previous contact lens use was 2.64 ± 2.41 years. The questionnaire results showed that OOK lenses were more comfortable to wear than conventional lenses. Significant changes in spherical equivalents, keratometry, and central corneal thickness were observed at 6 weeks compared with baseline values (all p < 0.001). No significant changes were detected in ocular surface or meibomian gland status at 1 and 6 weeks compared with baseline values (all p > 0.05). Conclusions: Based on the findings, OOK is a relatively safe modality with good compliance for normal myopic adults experiencing contact lens discomfort.
