Fukuyama Congenital Muscular Dystrophy in a Young Infant

국문 초록

Fukuyama 선천성 근이영양증은 일본에서 흔히 발생하는 상염색체 열성 유전 질환으로서, 영아기에 발병하는 근긴장 저하와 발달 장애를 초래하는 것이 특징적인 임상 소견이다. 염색체 9q31에 위치한 fukutin gene의 돌연변이에 의한 α-dystroglycan의 글리코실화 반응 저하로 인해 근이영양증 및 눈과 뇌의 이상을 초래하는 것으로 알려져 있다. Fukutin gene의 돌연변이는 α-dystroglycan의 글리코실화 반응 결함으로 인한 선천성 근이영양증 중 가장 흔하며, 3' UTR의 heterogygous RT insertion 돌연변이와 novel pseudoexon 돌연변이가 가장 흔한 것으로 한국에서는 보고 되었다. 본 증례에서는 생후 5개월에 근긴장 저하를 주증상으로 내원한 영아에서 혈청 creatine kinase의 증가 및 뇌 MRI를 통해 대뇌피질의 이형성증, 소뇌이형성증 및 대뇌백질 변화 등을 보여 Fukuyama 선천성 근이영양증을 의심하였으며, 면역조직화학 검사를 통해 α-dystroglycan의 글리코실화 반응 결함을 확인하였고, 유전자 검사를 통해 fukutin gene의 heterozygous RT insertion 돌연변이를 확인하여 한국에서 분자 유전 검사로 확진 된 최연소 사례로 보고하는 바이다.

영문 초록

Fukuyama congenital muscular dystrophy is an autosomal recessive disorder characterized by muscular dystrophy with central nervous system and ocular malformation. Fukutin gene mutations are the most common genetic cause of congenital muscular dystrophy with α-dystroglycanopathy and compound heterozygosity of the retrotransposal insertion; the novel pseudoexon mutation is the most prevalent genotype in Korea. We report the youngest documented Korean case of Fukuyama congenital muscular dystrophy, in a 5-month-old boy who presented with weak sucking power, muscle weakness and hypotonia. The findings of brain magnetic resonance imaging were specific to Fukuyama congenital muscular dystrophy and molecular analysis revealed compound heterozygosity of retrotransposal insertion and deep intronic mutation in the fukutin gene.
