노인성 치매환자의 주 부양자가 지각하는 심리적 부담 및 부양의무감의 비교 문화적 연구

국문 초록

본 연구는 가정에서 치매환자를 돌보는 부양자들이 지각하는 심리적 부담이나 부양의무감을 비교 문화적으로 살펴보았다. 한국인, 재미교포, 및 백인 부양자들을 비교했을 때 부양자들의 특성에서부터 문화적 차이가 드러났다. 곧 한국인은 며느리들이 대부분 부양자이었던 반면, 백인들은 배우자나 딸이 그 역할을 담당하고 있었다. 불안 장애나 우울증을 비교하였을 때 한국인이 가장 심하게 나타났으며, 그리고 백인들이 가장 낮은 점수를 보였다. 그러나 연령이나 교육수준, 신체적 건강 등의 공변인의 중재 효과를 고려했을 때에는 우울증상의 차이가 사라졌다. 공변인의 효과에도 불구하고 불안의 차이는 사라지지 않았는데, 한국인들의 불안 증상이 가장 심하게 나타난 점은 치매 현상이 최근에 사회적인 관심을 받기 시작한 것이나 치매에 대한 지식을 충분히 갖추지 못한 것과 관련될지도 모른다. 부양상황에 대한 주관적인 부담은 예상과는 달리 재미교포 부양자들에서 가장 높게 나타났다. 이러한 결과는 재미교포들이 문화적으로 다른 미국에서 살아가는 상황에 기인한 것 같다. 마지막으로 환자의 보호에 대한 의무감을 살피는 가족주의 척도에서는 기대한 바처럼 한국인들이 가장 높게 나타났고, 백인들에서 가장 낮았다. 이러한 결과는 의무감이 높은 한학인들이 다른 집단보다 치매환자의 부양 상황을 더 부담스럽게 지각하는 것으로 해석되었다.

영문 초록

Family caregiving for the cognitively impaired elderly patients can result in a number of negative problems such as psychosomatic disorders, mental health problems, limited social activities and so on. The main purpose of this study was to find out any cross-cultural differences in caregiving distress from the Korean, Korean-American, and White caregivers of the demented family members. Some of those differences were from the relationships between the caregivers and their parents. That is, most of the Korean caregivers were daughters-in-law while many White caregivers were spouses and some daughters. General characteristics of the Korean-American were between the Korean and White groups. Among the three groups, the Korean caregivers reported that they have the least emotional as well as instrumental helpers while they have taken the caregiving roles. Moreover, the Korean caregivers reported their physical condition and functional abilities much poorer than the other two groups. The Korean caregivers showed higher levels of emotional distress such as anxiety or depression and more negative appraisal of their caregiving situation such as subjective burden than the white caregivers, and showed stronger filial obligation due to their tradition than the White group while that of the Korean-American was in between the Korean and White groups. Even if many ethnic group differences have been found in this study, some limitations should be noted before generalizing the results.
