한인 이민노인의 우울증에 영향을 미치는 요인

국문 초록

본연구의 목적은 한국이민 노인들의 우울증에 영향을 미치는 주요 위험요인 및 완충요인들을 밝히는데 있다. 연구 자료 수집을 위해 뉴욕 주의 주요 도시에 거주하는, 인지적 능력이 정상인 55세 이상의 재가노인 234명을 면담 조사하였다. 우울증의 다중요인을 평가하는 이론적 틀로서 George의 Social Antecedent Model (1994)을 적용하여 네 그룹의 외생 변수들이 선정되었다. 즉, 1) 인구사회학적 요인, 2) 이민 관련 및 문화적응 요인, 3) 신체건강 및 생활사건요인, 4) 사회적지지 및 대처요인들이다. 통계분석방법으로 heteroskedasticity resistant (HR) 로버스트 알고리듬을 위계적 방법에 접목시켜 수행함으로써 다중회귀분석 결과의 신뢰도를 높였다. 연구 결과는 연구표본의 약 40%가 우울증을 겪고 있음을 보여주고 있다. 우울증의 주 영향요인으로는 연령, 수입, 이중 언어 실력(Bilingualism), 스트레스성 생활사건, 주관적 건강상태, 사회적지지만족도, 우울증 치료경험이 밝혀졌고, 중요한 상호작용요인으로는 스트레스성 사건들X주관적 건강상태와 주관적 건강상태X사회적 지지 만족도로 밝혀졌다. 이 연구는 우울증 예방과 우울한 경향을 보이는 노인들을 확인, 발견, 의뢰할 수 있는 프로그램 및 치료서비스 개발에 구체적인 방안을 제시하고, 사회복지 정책적 함의 및 향후 연구 방향을 제안하고 있다.

영문 초록

The overarching objective of this study is to empirically investigate principal bio-psycho-social risk factors associated with depression among elderly Korean immigrants in the U.S.. Data were collected from a survey of 234 community-dwelling cognitively competent Korean immigrants (age≥55) residing in the New York metropolitan area. Due to the lack of a theoretical model to assess depression, the study utilized an adaptation of George's Social Antecedent model of depression (1994) from which four sets of exogenous variables were selected. The statistical methodology employed a robust (heteroskedastically resistent (HR)) regression, combined with a hierarchical inclusion modeling strategy, to obtain reliable results. The results indicated a high prevalence rate of depression (40%) in the sample of Korean immigrant elders studied. From the robust findings. significant explanatory factors were found to include age, income, bilingualism, stressful life events, self-perceived health status, perceived satisfaction with social support, prior treatment for depression within past 5 years. Also noteworthy were two significant interactions including perceived health status by life events and perceived health status by satisfaction with social support. The results have important implications for social work practice, policy and directions for future research.
