요양병원 노인환자의 우울 측정도구 개발

국문 초록

본 연구는 요양병원에 입원한 노인환자의 우울을 측정할 수 있는 도구를 개발하고자 요양병원에 입원한 노인환자의 우울의 속성을 규명하여, 간호사의 객관적인 관찰과 지속적인 사정을 통해 노인 환자의 우울을 감별할 수 있는 우울 도구를 개발하여 검정하고자 시도되었다. 도구의 타당도와 신뢰도 검정은 2005년 10월 7일부터 11월 2일까지 요양병원에 입원한 701명의 노인환자를 대상으로 130명의 간호사가 측정한 자료를 분석에 이용하였다. 본 연구에서 개발한 우울 도구의 타당도 검정 방법은 내용타당도, 구인 타당도, 동시 타당도를 실시하였고, 신뢰도 검정방법은 내적일관성 신뢰도, 검사-재검사 신뢰도를 실시하였다. 본 연구의 주요 결과, 정서적 불편감, 신체적 약화 및 수면장애, 심리사회적 위축, 슬프거나 불안ㆍ초조 등 4개영역 총 13문항의 요양병원 노인의 우울측정 도구가 개발되었다. 연구결과 요양병원 노인환자의 우울 속성이 반영된 문항을 개발하고 그 타당도와 신뢰도를 검정하여 요양병원에 입원한 노인환자의 우울증상을 조기에 발견되고 관리할 수 있는 도구로 개발하였다. 따라서 본 도구를 요양병원에서 활용한다면 잠재된 노인의 우울을 객관적으로 사정하고 측정하여 간호계획과 평가를 수행할 수 있을 것이며 나아가 장기요양 노인환자의 삶의 질 향상에 기여하리라 사료된다.

영문 초록

This study is a methodological research to develop a scale to measure the level of depression of the Korean elderly inpatients. The data collection for validity and reliability tests was done during October 7th and November 2nd by the comprehensive survey research of 701 elderly inpatients in 27 long-term care hospitals participating in the long term care hospital national health insurance project. The validity and reliability test for this study includes factor analysis, concurrent validity test, and the validity test consists of internal-consistency Cronbach Coefficient Alpha and test-retest reliability test. As the result of 13 items, factor is named as emotional, physical, Psycho-social, behavioral. When analyzing the four factors, it explained the 60.8% of the total variance. The Pearson coefficient for the developed depression rating scale and K-GDS was found to be r=0.53(p=0.0001), Cronbach's a of the over-all internal consistency(Cronbach'α=0.89), and test-retest reliability scored 0.97. Conclusively, the depression rating scale for the elderly inpatients in the long-term care hospital developed in this study has passed a variety of validity and reliability tests and may be applied to measure the degree of the depression of the elderly inpatients.
