국문 초록

노인의 우울증은 인식되지 않고 치료받지 않는다면 생명에 위협적일수 있는 중요한 건강상의 문제이다. 상당한 관심에도 불구하고, 노인성 우울증의 유병율에 관한 일치된 결과는 없다. 본 연구는 65세 이상 노인에서 주요 우울장애의 유병율을 조사하고 이를 지역사회 정신건강 프로그램을 위한 기초자료로 사용하기 위한 것이다. 방 법： 경기도 과천시에 거주하는 65세 이상 노인 500명을 대상으로, 숙련된 조사원들이 DSM-Ⅳ에 따른 반구조화된 면담을 실시하고 Geriatric Depression Scale(GDS), Korea Depression Scale(KDS), MMSE-K, Korea-Dementia Rating Scale(K-DRS), 등의 검사를 실시하였다. 결 과： 표본에서 경도, 중등도 및 심도의 우울증상의 유병율은 각각 18.8%, 7.4%, 17.8%이었으며, 주요 우울장애의 유병율은 7.8%이었다. 성별, 연령, 학력, 가족 유형과 같은 사회인구통계학적 변인에서 우울군과 비우울군 간에 유의한 차이는 발견되지 않았다. 주요 우울증은 신체질환, 스트레스가 되는 생활 사건, 및 인지기능의 장애와 상관이 있었다. 결 론： 본 연구의 결과는 다른 역학조사의 유병율과 유사한 양상을 보여준다. 우울군은 비우울군에 비해 심리적 고 통, 생활사건 및 인지기능의 장애가 유의하게 더 많음을 보여주었다. 우울증의 정확한 조기 진단과 적절한 치료는 양호한 예후에 영향을 미친다.

영문 초록

Objective：Depression in the elderly is a major health concern that can be life threatening if not recognized and treated. Despite considerable interest, there in no consensus regarding the prevalence of depression in later life. This study was aimed to investigate the prevalence of major depressive disorder in the population over age 65 years and provide fundamental data for community mental health programs. Method：500 elderly people aged over 65 years and oner in Kwachon were interviewed according to DSM-Ⅳ and examined Geriatric Depression Scale(GDS), Korea Depression Scale(KDS), MMSE-K, Korea-Dementia Rating Scale(K-DRS), etc. by trained researchers. Results：Of the sample, prevalence of mild, moderate and severe depressive symptoms are 18.8%, 7.4%, and 17.8%, respectively. Prevalence of major depressive disorder is found to be 7.8%. There is no significant differences between depressive & non-depressive groups in the sociodemographic variables such as sex, age, education, family type, etc. Major depressive disorder is associated with physical illness, stressful life events, and cognitive dysfunction. Conclusion：The results showed similar prevalence rates to other epidemiological studies. Depressed group showed more psychological distress, stressful life events and cognitive dysfunctions than non-depressed group. Accurate and early diagnosis and adequate treatment favor a good prognosis.
