삼차원 동작분석과 내담자 중심(client-centered)의 일상생활동작평가를 통한 편마비환자의 강제유도운동치료효과

국문 초록

본 연구의 목적은 편마비 환자를 대상으로 실시한 강제유도운동치료(Constraint- Induced Movement Therapy; CIMT)가 삼차원 운동속도 평가와 캐나다 작업수행 평가를 이용한 일상생활동작에 효과를 입증할 수 있는지를 알아보기 위해 실시되었다. 추가적으로 대상자의 상지기능은 상자와 나무토막 검사, 9공 막대검사 그리고 악력(grip strength)을 측정하였다. 본 연구는 개별실험 연구(single subject design) (ABA design)였으며, 강제유동운동치료를 2주 동안 건측을 깨어있는 시간의 90% 동안 장갑을 끼워서 사용을 억제하고, 6시간 동안 환측을 집중적으로 치료하는 방법으로 실시하였다. 본 증례를 통하여 강제유도운동치료 2주후에 뇌졸중환자의 운동속도, 악력, 일상생활수행능력이 향상되었다. 추후 연구에서는 이러한 결과를 더 많은 대상자로 확대할 필요가 있을 것이다.

영문 초록

The purpose of this study was to investigate the effect of constraint-induced movement therapy (CIMT) using 3D kinematic analysis and activities of daily living (ADL) by Canadian Occupational Performance Measure(COPM) for an individual with hemiparesis. In addition, motor function of the subject's affected upper extremity was assessed by Box and Block Test, 9-hole pegboard test, and grip strength. This study was a single-subject ABA research design. The use of unaffected upper extremity was restrained during 2 weeks CIMT by appling mitt for 90% of waking hours. Additionally, the subject were taken 6 hours/day of intensive training of the affected upper extremity. The movement time, grip strength, and performances in ADL were improved after 2 week CIMT, The results of this case study suggests that CIMT appears to be an effective approach in improving function of the affected upper extremity after the stroke. Further study with larger sample size is required to validate the findings of this case study.
