국문 초록

In adult patients with skeletal class II malocclusion with severe skeletal discrepancy, surgical orthodontic treatment can provide functional and aesthetic results. Depending on the patient’s preference and the mandibular condyle, 2-jaw surgery, 1-jaw surgery, or anterior segmental osteotomy may be an option. This 25-year-old female patient presented with a protrusion of the lips, excessive overjet with no missing teeth and retrusive mandible. She had no complaints of noise or pain of the TMJ, but radiographs showed flattening of the mandibular condyle. The patient was diagnosed with skeletal class II malocclusion, and orthodontic treatment with surgery was decided, with anterior segmental osteotomy based on consideration of the mandibular condyle and patient choice. Considering the patient’s preference and the class I relationship of the molars at the initial records, she was treated with clear aligner treatment. After 12 months of treatment, functional occlusion was achieved, and the patient’s complaints of retrusive chin and lip protrusion were improved. During the 13-month maintenance period, stable treatment results confirmed that clear aligner treatment with anterior segmental osteotomy can be an appropriate treatment option in adult patients with severe skeletal class II malocclusion.
