고령자의 고독과 외로움 평가도구에 관한 체계적 문헌고찰: 경향과 활용전망

국문 초록

목적 : 본 연구는 기존의 고령자의 외로움 및 사회적 고독감 평가도구들에 대한 문헌고찰을 통해 각 문헌의 질적 수준, 평가도구의 종류 및 빈도, 특성을 분석하고자 한다. 이 평가는 고령자의 외로움 및 사회적 고독감을 평가하기 위한 새로운 평가도구를 개발하는 기반으로 활용될 것이다. 연구방법 : 검색기간은 2013년 1월부터 2023년 8월까지였으며, 국외 데이터베이스 Cumulative Index of Nursing and Allied Health Literature, Google Scholar, PubMed, Web of Science를 사용하여 문헌을 수집하였다. 주요 검색어는 “older adult” OR “elderly” AND “loneliness” OR “social isolation” AND “assessment” OR “measurement”를 사용하였다. 선정기준과 제외기준에 따라 최종 65건의 문헌이 선정되었다. 결과 : 최종 문헌으로 선정된 65건을 고찰한 결과, 총 9개의 평가도구가 고령자의 외로움과 고독을 평가하기 위해 사용되었다. 평가방식은 자가보고 혹은 자기기입식의 설문지 형태로 구성되어 있었다. 평가항목은 개인의 정서적 측면, 환경적 측면, 관계적 측면을 주로 측정하는 것으로 나타났다. 평가문항은 최소 6문항부터 최대 37문항으로 다양하였으며, 점수척도는 리커트척도(범위: 3~7점)를 적용하여 측정하는 것으로 나타났다. 결론 : 본 연구의 결과는 고령자를 위한 새로운 평가도구를 개발하는 데 있어서 다양한 측면에서의 평가항목 및 점수 척도를 고려하는 데 중요한 기초자료로 활용될 것이다. 더불어 추후 이를 기반으로 개발된 평가도구는 고령자의 외로움과 사회적 고독을 정량적으로 평가하여 상태를 종합적으로 이해하고 정량화함으로써 정확한 중재 및 지원 프로그램을 개발하고 운영할 수 있을 것이다.

영문 초록

Objective : This study aims to analyze the theoretical basis and key evaluation factors of each tool through a literature review of existing loneliness and social isolation assessment tools. This evaluation can be used as the basis for the development of new assessment tools. Methods : Literature was collected from overseas Cumulative Index of Nursing and Allied Health Literature, Google Scholar, PubMed, and Web of Science databases. The primary search keywords used were “older adult” or “elderly,” “loneliness” or “social isolation,” and “assessment” or “measurement.” Based on the selection and exclusion criteria, 65 studies were selected. Results : After reviewing 65 cases selected as the final literature, a total of 11 evaluation tools were used to analyze the loneliness and social isolation of older adults. The evaluation method consisted of self-reported and self-written questionnaires. The evaluation items were found to mainly measure individual emotional, environmental, and relational aspects. The evaluation questions ranged from a minimum of 6 to a maximum of 37, and the score scale was measured using a Likert scale (range: 3~7 points). Conclusion : The results of this study can be used as important basic data for considering evaluation items and scoring scales in various aspects of the development of new evaluation tools for older adults. Additionally, evaluation tools developed based on this will be able to develop and operate accurate intervention and support programs by quantitatively evaluating the loneliness and social isolation of older adults and comprehensively understanding and quantifying their condition.
