안면비대칭을 동반한 골격성 III급 부정교합 환자에서 반복적 상악골 확장을 통한 상악 전방부 발육 치험례

국문 초록

목적: 본 증례는 후기 청소년기에 발생한 안면 비대칭을 동반한 골격성 III급 부정교합을 개선하기 위해 반복적인 미니스크류 보강형 급속구개확장 (MARPE)을 통한 상악 전방부의 발육을 이용하여 성공적인 비수술 치료 접근 방법을 보여주었다. 임상 증례: 15세 여성 환자가 전치부 반대교합과 견치 이소 맹출을 주소로 교정 치료 상담을 위해 내원하였다. 임상 및 방사선적 평가를 토대로 심각한 상악의 횡적 부조화 및 안면 비대칭을 동반한 과발산형의 골격성 III급 부정교합으로 진단하였다. 횡적 부족을 해결하기 위해 상악 악궁에 두 차례의 확장을 시행하였으며, 이로 환자의 환자 안모의 미적 향상과 안면 비대칭이 현저하게 개선되었다. 상악 전방부의 발육과 전략적으로 비대칭적 후방 이동 및 전치부 기울기 조절을 포함한 포괄적 교정치료가 시행되었다. 30개월의 치료기간 후, 전치부 반대교합 및 견치의 이소맹출이 개선되었으며, 안모의 미적 향상과 안면비대칭 및 교합의 개선이 이루어졌다. 환자는 치료 결과에 현재 만족하며 유지 관리 중에 있다. 결론: 횡적 부조화의 효과적인 조절은 골격성 III급 부정교합과 관련된 전후방적 문제 해결에 중요한 역할을 한다. MARPE는 후기 청소년기 혹은 젊은 성인들에게 효과적인 접근 방법으로 입증되었다. 상악 전방부의 발육과 하악 후방 회전이 동시에 이루어지면서 본 증례의 비수술 교정치료는 환자의 조화로운 안모의 결과를 창출하였다.

영문 초록

Purpose: This case report demonstrated a successful non- surgical management approach using premaxillary devel-opment through repeated maxillary expansion to correct a skeletal Class III malocclusion with facial asymmetry in late adolescence. Clinical Case: A 15-year-old female patient sought orthodon-tic consultation due to complaints of anterior crossbite and ectopic erupted canines. Clinical and radiographic evaluations revealed a hyperdivergent skeletal Class III malocclusion with significant transverse deficiency and facial asymmetry. To ad-dress the patient’s severe transverse deficiency, the maxillary arch underwent two rounds of expansion, resulting in remark-able improvements in the patient's facial profile esthetics and facial asymmetry. A comprehensive orthodontic approach was adopted, incorporating premaxillary development, strategic asymmetrical intrusive distalization along with incisor inclina-tion management. After 30 months of active treatment, suc-cessful correction of the anterior crossbite and ectopic canines was achieved, along with significant improvements in facial profile esthetics, facial asymmetry, and occlusion. The patient expressed satisfaction with the treatment outcomes and is un-dergoing retention follow-up. Conclusion: Effectively managing transverse discrepancies plays an important role in achieving successful correction of anteroposterior problems associated with Class III malocclu-sion. MARPE proves to be an effective approach, particularly for late adolescence or young adults. By incorporating premax-illary development and mandibular backward rotation, the camouflage orthodontic treatment successfully resulted in a harmonious facial profile for the patient.
