경도인지장애 및 치매노인을 위한 작업치료 효과에 관한 체계적 고찰

국문 초록

목적 : 본 연구에서는 경도인지장애 및 치매노인의 실제 활동과 참여 수준을 높일 수 있는 작업치료 중재 방법에 대하여 체계적으로 고찰하고자 한다. 연구방법 : 2001년부터 2018년까지의 논문을 대상으로 “Occupational Therapy” and “dementia” or “Mild Cognitive Impairment” and “participation”, “작업치료” and “치매” or “경도인지장애” and “참 여”를 검색용어로 검색하였다. 국외 논문은 MEDLINE, EMBASE, cochrane CENTRAL, PubMed, 국 내 논문은 KISS, RISS, NDSL, KMBASE, KiSTi를 이용하여 검색하였다. 결과 : 최종 선정된 8편의 연구에서 질적 수준은 해외 연구가 국내 연구에 비하여 높은 수준이었으며 중 재대상으로는 알츠하이머형 치매가 가장 많았다. 작업치료 중재방법은 지역사회 작업치료와 환자중심의 접근이 많았으며 가정에서 중재가 많이 이루어지고 있었다. 그리고 참여보다 활동을 측정한 연구가 많았 으며 활동과 참여와 함께 가장 많이 측정된 영역은 인지기능이었다. 결론 : 경도인지장애 및 치매노인을 대상으로 한 활동과 참여 증진을 위한 작업치료가 증가하고 있으며 작 업치료 환경이 병원에서 지역사회로 발전하면서 환자중심의 치료가 이루어지고 있지만 아직까지 부족한 실정이다. 앞으로는 활동과 참여 증진을 위한 연구가 활발히 이루어져야 할 것이다.

영문 초록

Objective: This study aimed to systematically review occupational therapy intervention methods for an improvement in the activity and participation of persons with mild cognitive impairment or dementia. Method: The search terms, “Occupational therapy” and “Dementia” or “Mild Cognitive Impairment” and “Participation,” “작업치료” and “치매” or “경도인지장애” and “참여,” were used to search the literature published between 2001 and 2018. For the international studies, we used MEDLINE, EMBASE, cochrane CENTRAL, and PubMed, and for Korean studies, KISS, RISS, NDSL, KMBASE, and KiSTi were used. Result: From the eight papers selected, international studies were found to be of higher quality than Korean studies. The most common intervention targets were persons with Alzheimer's type dementia. Common occupational therapy interventions were community occupational therapy and client-centered approaches. Interventions were noted to often take place in a home environment. There were a larger number of studies on measuring the level of activity compared to studies measuring the participation. Along with activity and participation, cognitive function was the most measured area. Conclusion: In this study, occupational therapy intervention methods to improve the activity and participation of persons with mild cognitive impairment or dementia are increasing in number, and a client-centered approach is being made as the occupational therapy environment extends from hospitals to communities, although to an insufficient degree. Thus, more research is needed to improve the activities and participation of persons with mild cognitive impairment or dementia.
