중·고령자의 노쇠 발생에 영향을 미치는 요인에 관한 연구

국문 초록

노쇠는 기능 저하나 장애, 입원 및 사망 등으로 이어지는 주요 공중 보건 문제이다. 본연구는 고령화연구패널조사를 이용하여 중․고령층의 노쇠와 관련된 요인을 확인하고자 하였다. 자료 분석은 우선 수년에 걸친 조사자료를 모두 하나로 합한, 즉 횡단적 자료를 토대로 노쇠에 영향을 미치는 요인(인구통계학적, 사회경제적, 사회적 기능, 건강행태, 그리고 건강 상태 요인)의 효과를 확인하고, 또 그 조사자료를 측정 시점의 시간적 차이를 두어 종단적 접근 방식에 의해 요인들의 효과를 살펴본 것이었다. 횡단적 자료를 토대로 분석했던 결과, 노쇠가 인구학적 요인(남성, 고령층, 배우자가 없는 자, 그리고 농촌지역 거주자), 사회경제적 요인(저학력, 저소득, 그리고 경제적 활동이 없는 자), 사회적 기능 요인(친한 친구와의 만남 횟수 및 사회활동 참여), 건강행태 요인(음주자 및 규칙적인 운동을 하지 않는 자), 그리고 건강 상태 요인(만성질환 개수, 우울, 인지 능력, 일상생활 수행능력평가, 그리고 도구적 일상생활 수행능력평가)과 관련됨을 확인했다. 또 종단적 성격의 자료를 살펴본 결과, 앞에서 언급한 횡단적 성격의 자료 분석 결과와 거의 유사했다. 본 연구의 결과는 사회적, 경제적 또는 신체적으로 취약한 노인들을 공중 보건 개입의 우선 대상자로 선정하고 지원할 필요가 있음을 시사한다.

영문 초록

Frailty is a major public health problem leading to functional decline or disability, hospitalization, and death. This study aimed to identify factors related to frailty in the middle-aged and old age groups using the Korean Longitudinal Study of Aging. Data analysis first was to verify the effects of factors influencing frailty (demographic, socio-economic, social function, health behavior, and health status factors) based on cross-sectional data that integrated survey data over several years into one. Furthermore, the effect of the factors was examined in a longitudinal approach with a time difference in the time of measurement of the survey data. As a result of cross-sectional data analysis, demographic factors (male, elderly, single, and rural residents), socio-economic factors (people with low education, low income, and no economic activity), social function factors (number of meetings with close friends and/or participation in social activities), health behavior factors (drinkers and people who do not exercise regularly), and health status factors (number of chronic diseases, depression, cognitive ability, ADL, or IADL) were confirmed to be related to frailty. In addition, as a result of examining the effect on frailty with longitudinal data, it was almost similar to the results of the cross-sectional data analysis mentioned earlier. The findings of this study suggest that it is necessary to prioritize and support public health interventions for the older adults who are socially, economically, or physically vulnerable. It is suggested that the intervention program should include counseling and education related to improvement of willpower for recovery from frailty, and training related to improvement of physical function to perform daily life independently rather than improvement of a specific single indicator.
