주의력결핍 과잉행동장애(ADHD) 진단을 받은 아동의 약물치료에 따른 어머니의 ‘마음갉기’경험

국문 초록

이 연구는 ADHD로 진단을 받은 아동의 주 양육자인 어머니에게 있어 자신의 아이가 ADHD로 진단을 받은 시점에서부터 아이에게 약물치료를 선택하게 된 계기와 그 과정에서의 ‘마음갉기’ 경험을 심층적으로 탐색하였다. 본 연구는 질적 사례연구로 Mad Studuies의 분석 틀을 기반으로 하여 연구에 참여한 ADHD 진단을 받은 아동의 약물치료를 경험한 어머니 5명에 대한 인터뷰를 통해 얻은 자료를 분석한 결과, 최종적으로 43개 의미단위, 17개의 하위범주, 8개의 범주, 3개의 구분으로 도출되었다. 분석 결과를 근거로 아래와 같은 제언을 하였다. 실천적 함의로 부모의 정서적 고통에 대한 사회적 지지와 부모교육이 필요하다. 정신건강의학과 진단 및 처방에 있어 낙인효과를 방지할 수 있도록 개인 정보의 보호가 절실하다. 사회적 함의는 ADHD 진단을 받은 아동의 약물치료의 실체에 관한 엄밀한 고찰과 비판적 성찰, 그리고 무엇보다도 ADHD 진단 아동에 대한 긍정적인 면을 찾는 시각이 불가결하다. 더불어 다양성의 일면으로 ADHD의 존중과 인간의 다양한 측면의 하나로 인정해야 함도 중요하다. 또한, ADHD의 진단방식에 있어 전문가의 창의적인 진단이 요구된다. 본 연구에서 밝힌 어머니의 마음갉기 경험은 정신장애 영역에서의 장애학적 시각과 정확히 맥을 같이 한다. 이것이 본 연구와 관련되는 지점은 ADHD에 대한 인식변화의 깨달음은 Mad Studies적 시각으로 표현되는 장애학적 패러다임으로의 전환을 시사하는 것이다

영문 초록

This study had aimed to promote a new ADHD view and social welfare suggestions by in-depth exploration of the opportunity for a mother, the main caregiver of child diagnosed with ADHD, to obtain critical knowledge of the existing and current pathological treatment of ADHD. This study analyzed data obtained through interview with five mothers who experienced drug treatment of children diagnosed with ADHD based on Mad Studies analysis framework, and finally derived 43 semantic units, 17 subcategories, 8 categories, and 3 categories. Based on the analysis results, the following suggestions were made. In practical implications, social support and parent education for emotional distress of parents are needed. Personal information protection is urgently needed to prevent stigma effect in the diagnosis and prescription of mental health medicine. Social implications include rigorous consideration and critical reflection on the substance of drug therapy in children diagnosed with ADHD, and above all, the view of finding positive aspects of children diagnosed with ADHD is indispensable. In addition, it is important to recognize ADHD as one of the aspects of diversity and respect for human beings. In addition, it is also necessary to make a creative diagnosis of experts in the diagnosis method of ADHD. The mother s mind-reading experience revealed in this study is exactly in line with the disability perspective in the area of mental disorder, while the realization of the change in perception of ADHD suggested a shift to a disability paradigm expressed in Mad Studies perspective.
