치매환자와 주 부양자의 관계에 따른 돌봄 부담 요인

국문 초록

목적 : 본 연구의 목적은 치매안심센터를 이용하는 주 부양자의 특성별 맞춤형 프로그램과 지원방안을 제시하고자 하며, 치매정책 사업지침 내 가족지원사업의 기초자료를 마련하고자 한다. 연구방법 : 보건소 치매안심센터에 등록된 치매환자의 주 부양자를 대상으로 하였다. 대상자의 일반적 특성 및 우울의 정도와 치매태도, 부양부담, 치매환자의 정신행동증상 여부를 포함한 자가기입 돌봄부담 분석을 활용하여 측정하였다. 측정도구는 각각 Patient Health Questionnaire-9: PHQ-9와 Dementia Attitude Scale: DAS, Short Zarit Burden Inventory: S-ZBI로 평가하였고, 치매환자의 정신행동증상 여부는 Neuropsychiatric Inventory Questionnaire: NPI-Q를 통해 치매환자의 심한정도 및 보호자의 고통정도를 알아보고자 하였다. 또한, 치매안심센터 내 가족지원사업의 서비스 요구도를 5가지 명목척도로 조사 하였다. 연구조사 방법은 우편 및 대면 설문을 통해 진행하였다. 연구 목적에 대한 설명 및 동의를 포함한 설문지를 총 423부를 배부하여 이 중 152부를 회수하였고, 응답에 충실하지 못한 41부를 제외하여 최종 111부룰 대상으로 비교⋅분석하였다. 연구결과 : 치매환자와 주 부양자의 관계에 따른 돌봄부담을 비교한 결과 환자의 정신행동증상여부 (p<.05)에서 배우자 외 집단에서 유의하게 높은 차이를 보였다. 또한, 치매환자의 특성과 부양자의 돌봄 부담에 관한 영향요인을 파악하고자 환자와의 관계를 중심으로 조절 효과를 확인한 결과, 환자의 연령과 부양자 간의 관계가 치매태도에 있어서 상호작용하는 것으로 나타났다. 결론 : 치매안심센터 내 가족지원사업 운영 추진 시 주 부양자의 인구학적 특성 및 환자와의 관계에 따른 맞춤형 프로그램이 요구된다.

영문 초록

Objective : The purpose of this study is to collect basic data necessary to propose customized programs and support plans for each characteristic of the Caregiver s who use the Center for dementia. Methods : The subjects were the Caregiver of dementia patients registered at the Center for dementia at the public health center. The general characteristics and depression of the subjects were evaluated as Patient Health Questionnaire-9: PHQ-9, Dementia Attitude Scale: DAS, and the Caregiver’s support burden was evaluated as Short Zarit Burden Inventory: S-ZBI, and whether Behavioral and psychological Symptoms of Dementia Scale (Neuropsychiatric Inventory Questionnaire, NPI-Q) to determine the severity of dementia patients and the degree of pain of caregiver. In addition, a survey was conducted on the service needs of the family support project of the Center for Dementia, The survey was conducted through mail and face-to-face interviews. A total of 423 copies were distributed and 152 of them were recovered, and the final 111 copies were compared and analyzed, excluding 41 copies that were not faithful to the survey. Results : As a result of comparing the burden of care according to the relationship between dementia patients and their caregiver, it was analyzed that there was a significant difference in Behavioral and psychological Symptoms of Dementia (p<.05). In addition, as a result of identifying the characteristics of dementia patients and the factors influencing the care burden of dependents and confirming the moderating effect centering on the relationship with the patient, it was analyzed that there is an interaction according to the patient s age. Conclusion : The Center for dementia needs to organize a program suitable for the characteristics of the caregiver of dementia patients, and for this purpose, the relationship with the patient should be considered.
