노인장기요양보험제도 내 시각장애인 안마급여 도입 가능성 고찰: 일본 개호보험제도를 중심으로

국문 초록

본 연구는 불안정한 노동시장에 놓여 있는 시각장애인 안마사들의 안정된 취업처를 모색하기 위해 노인장기요양보험제도에서의 안마서비스 도입을 고찰하는데 그 목적이 있다. 이를 위해 우리나라 보다 고령사회를 먼저 맞이하여 노인요양의 문제를 대처한 일본의 개호보험 사례를 중심으로 문헌분석을 실시하였다. 연구의 내용은 다음과 같다. 먼저 국내 시각장애인 안마업의 현황과 문제를 살펴보고, 다음으로 일본의 개호보험제도에서 제도개요, 급여, 안마사관련 서비스제공인력 등을 중심으로 분석하였다. 이를 통해서 노인장기요양보험제도에서의 안마서비스 도입을 위한 정책적 방향을 제시하고자 하였다. 연구결과, 일본의 경우 요개호고령자의 증가와 개호가족의 부담을 완화할 목적으로 2000년에 개호보험을 도입하였다. 개호보험 도입이후 요개호고령자가 급속히 증가하였고 특히 이들의 중증화가 되면서 의료비 증가 등으로 재정적 압박을 받게 되었다. 일본정부는 재정부담을 완화하기 위해 여러 차례 개호보험을 개혁을 단행하면서 재활서비스, 예방차원의 급여, 지역밀착형 급여 등을 도입하여 고령자의 중증화 속도를 늦추기 위한 예방노력을 하고 있다. 또한 요개호자의 증가에 따라 요구되는 개호인력 수급문제를 해결하기 위해서 개호사회복지사, 케어매니저, 기능훈련지도원 등에 안마사 등 다양한 보건의료 인력풀제도를 마련하여 개호인력 수급에 대처하고 있다. 2017년에 고령사회로 진입한 우리나라도 고령 사회에서 나타날 요양노인들의 중증화 현상에 맞춰 요양예방체계 구축이 시급한 과제이다. 이를 위해 일본에서 시행하여 있는 안마급여와 관련된 재활서비스, 케어매니저 및 기능훈련서비스 도입을 적극적으로 고민해야한다. 이러한 제도를 도입함에 있어 사회적 연대차원에서 시각장애인 안마사 등 사회적 취약계층을 위한 일자리 창출과 사회적 안전망구축에 대한 지혜도 함께 발휘해야 할 것이다.

영문 초록

The purpose of this study is to consider the introduction of massage services in the longterm care insurance system for the elderly in order to find a stable employment for visually disabled massage workers in an unstable labor market. To this end, a literature analysis was conducted focusing on the case of the long-term care insurance in Japan, which dealt with the problem of elderly care in the face of an aging society before Korea. The contents of the study are as follows. First, the actual status and problems of the domestic massage industry for the visually disabled were examined, and then, the Japanese long-term care insurance system focused on the outline of the system, the contents of benefit, and service providers. Through this, it was intended to present policy directions for the introduction of massage services in the long-term care insurance system for the elderly. As a result of the study, in the case of Japan, the long-term care insurance was introduced in 2000 with the aim of coping with the increase in the number of nursing care elderly and alleviating the burden on nursing families. Since the introduction of the long-term care insurance, the number of nursing care elderly has increased rapidly, and in particular, as their severity has increased, financial pressure such as an increase in medical expenses has come. In order to ease the financial burden, the Japanese government has reformed the long-term insurance several times, introducing rehabilitation services, preventive benefits, and local-friendly benefits to slow down the pace of seriousness of the elderly. In addition, in order to solve the problem of securing nursing staff due to the increase in the number of nursing caregivers, nursing social workers, care managers, and functional training instructors have prepared various health care manpower systems. In Korea, which has already entered an aged society in 2017, it is an urgent task to establish a care prevention system in line with the seriousness of the elderly who will be in the aged society. To this end, it is time to actively consider introducing rehabilitation services, care managers, and functional training services implemented in Japan. In introducing such a system, in terms of social solidarity and win-win, the wisdom of creating jobs and establishing a social safety net for the socially vulnerable, such as visually disabled massagers, should also be exercised.
