뇌졸중 환자에게 적용한 이중과제 훈련에 대한 체계적 고찰

국문 초록

목적: 본 연구는 뇌졸중 환자에게 제공한 이중과제 훈련의 최신 경향을 알아보기 위한 체계적 고찰 문헌연구로, 이중과제에 대한 근거를 제시하고자 한다. 연구방법: 본 연구는 2010년 1월부터 2019년 12월까지 출판된 문헌을 대상으로 국내 문헌은 Riss, Kiss에서 검색하였고, 국외 논문은 PubMed, Science Direct를 통해 검색하였다. 총 327편의 논문이 검색되었고, 포함 기준과 배제 기준을 바탕으로 분석하여 최종 12편의 논문이 분석에 사용되었으며 근거 수준 Ⅰ이 12편(100%)으로 분석되었다. 연구결과: 분석 결과 균형+상지가 4편 (33.4%)으로 가장 많았으며 보행+인지 3편(25.0%), 보행+상지 2편(16.7%), 보행+가상현실, 균형+인지+상지, 보행+상지+청각이 각 1편(8.3%)으로 확인되었다. 선정된 문헌에서 사용된 평가영역은 총 34개 영역이었으며, 동적 균형, 보행속도, 보행능력에 대한 평가가 많이 사용되었다. 결론: 본 연구는 뇌졸중 환자에게 제공한 이중과제 훈련의 사례들을 정리해서 공유할 수 있다는 것에 의의가 있다. 뇌졸중 환자의 이중과제 훈련에서 인지, 운동기능의 향상과 더불어 상호작용이 중요다는것을 알 수 있었으며 이중과제 수행에도 긍정적인 영향을 주는 것을 알 수 있었다.

영문 초록

Objective: The c urrent study was a systematic review through which we a imed to identify the latest trends in dual-task training provided to patients with stroke and to suggest a basis for dual-task training. Methods: From January 2010 to December 2019, domestic journals were searched using Riss and Kiss, and international journals were searched using PubMed, and Science Direct. A total of 327 papers were retrieved, and 12 papers were finally used in the analysis based on the inclusion and exclusion criteria. Basis level I was analyzed in all 12 papers (100%). Results: The analysis revealed that Balance + upper extremity(U/E) was the most common intervention type described in 4 papers (33.4%), followed by 3 papers reporting Gait + U/E (25.0%), 2 papers reporting Gait + U/E (16.7%), 1 paper about the Gait + Virtual reality program (8.3%), 1 paper describing Balance + Cognition + U/E (8.3%), and 1 paper reporting Gait + U/E + Auditory feedback (8.3%). Thirty-four areas of evaluation were reported in the selected papers, with many assessments of dynamic balance, gait speed, and gait ability. Conclusion: This study is meaningful in that cases of dual-task training provided to patients with stroke are summarized and shared. It was found that the interaction and improvement in cognitive and motor function are important, and they also have a positive effect on dual-task performance during dual-task training in patients with stroke.
