욕창예방방석 및 욕창예방매트리스 급여의 경제적 효과분석을 통한 장애인 보장구의 예방적 급여제도 실시 필요성에 대한 연구

국문 초록

국민건강보험공단의 2015년 11월 15일부터 2016년 3월 31일까지의 욕창예방방석과 욕창예방매트 리스를 지급 받은 가입자 1,059명의 지급일 전후 1년간 욕창 관련 진료비 현황을 분석하여 장애인 보장구의 예방적 급여 제도 실시 필요성을 제기하고자 한다. 국민건강보험공단의 장애인보장구 급여현황을 분석하며, 욕창예방방석과 욕창예방매트리스의 지급일 전후 1년간 총 진료비 변화를 분석하였다. 지급전에 비해 지급 후 총 진료비가 욕창예방방석은 20.73% 감소하였고, 욕창예방매트리스는 20.54%, 평균은 20.60%가 감소한 것으로 나타났다. 지급액 대비 절감액 배수는 욕창예방방석은 11.21배, 욕창 예방매트리스는 13.09배, 평균은 12.47배로 나타나 매우 비용효과적이고, 본인부담진료비 경감과 건강 보험 재정절감에 기여한 것으로 나타났다. 욕창예방방석과 욕창예방매트리스 급여의 경제적 효과 및 진료비 절감효과는 공단부담금과 본인부담금이 함께 감소하였으므로 장애인의 경제적 부담 해소뿐만 아니라 국민건강보험공단의 재정절감에도 기여한 것으로 조사되었다. 아울러 장애인 보장구 급여제도의 발전을 위해서는 장애인에게 양질의 제품이 제공될 수 있도록 하고, 건강보험 재정 누수를 방지하기 위하여 일원화된 공공전담기구가 필요하며, 급여확대와 함께 보다 강화된 급여기준과 사후관리가 필요하다. 항후 외국의 사례 및 저출산⋅고령사회 가속화에 따른 다양한 사회문제 발생이 예측됨에 따라 ICT(Information & Communication Technology; 정보통신기술)와 인공지능을 활용한 장애인보장구 개발 등 추후연구가 필요하다.

영문 초록

Objective : A total of 1,059 subscribers who received bedsore prevention cushions and pressure ulcer prevention mattresses from November 15, 2015 to March 31, 2016 of the National Health Insurance Corporation were analyzed for the treatment costs related to pressure beds for one year before and after the payment date. We want to examine the necessity of system implementation. Method : As the National Health Insurance Corporation analyzed the status of salaries of people with disabilities, and analyzed the changes in total medical expenses for one year before and after the payment of bedsore prevention cushions and pressure sickness prevention mattresses. Result : Compared with before payment, total medical expenses after payment decreased by 20.73% for the pressure ulcer prevention cushion, 20.54% for the pressure ulcer prevention mattress, and the average decreased by 20.60%. The amount of savings compared to the payment amount was 11.21 times for the pressure ulcer prevention cushion, 13.09 times for the pressure ulcer prevention mattress, and 12.47 times on the average, and it was very cost effective and contributed to the reduction of the medical burden cost and the health insurance financing. Conclusion : The economic effect and the reduction of the medical expenses of the bedsore prevention cushion and the pressure ulcer prevention bed reduction were both decreased not only the economic burden of the disabled but also the national health insurance corporation. In addition, in order to develop the benefit system for persons with disabilities, a unified public agency is needed to provide quality products to disabled people and to prevent health insurance financial leaks. In addition, Future studies such as the development of disabled people s protection using ICT(Information & Communication Technology) and artificial intelligence are also needed as the cases of foreign countries and the development of various social problems due to the aging of low birth rate and aging society are predicted.
