Completion Transoral Thyroidectomy Following Bilateral Axillo-Breast Approach Thyroid Lobectomy

Recent advances in remote-access surgical techniques for thyroidectomy, such as the transoral vestibular approach, transaxillary approach, and bilateral axillo-breast approach (BABA), have gained widespread acceptance. Among these, transoral surgery has been recognized for its safety, effectiveness, and cosmetic benefits. While research has been conducted on the feasibility of completion transoral thyroidectomy following initial transoral procedures, studies on completion transoral surgery after employing different approaches, particularly BABA, are significantly lacking. Here, we present 2 cases of completion transoral thyroidectomy performed following BABA lobectomy due to hyperthyroidism and a newly noted tumor, respectively. Despite fibrotic changes and adhesions from previous surgeries, flap dissection proceeded easily, and locating the midline of the strap muscle posed no significant challenges. Although initial adhesions were observed due to fibrotic tissue, they were easily separated without tracheal injury using a dissector. Both patients were safely discharged on the second postoperative day, with no incidences of vocal cord palsy or postoperative bleeding. Completion thyroidectomy via the transoral vestibular approach following BABA lobectomy is feasible, offering excellent cosmetic outcomes and minimal surgical challenges. These cases provide important insights into transoral thyroid surgeries, expanding the potential for surgical options.
