북한 이탈 주민의 정신건강

국문 초록

연구목적 : 북한 이탈 주민의 국내 입국 인원수는 지속적으로 증가하고 있다. 그러나 이들은 정착 과정에서의 어려움을 호소하고 있으며 정신건강 관련 증상의 유병률이 높을 것으로 생각된다. 따라서 일반적으로 유병률이 높은 북한 이탈 주민의 우울 및 불안 증상의 정도와 이에 영향을 미치는 요인과 정신건강 서비스 요구도에 대해 알아보고자 한다. 방 법 : 경기도 군포시에 거주 중인 성인 북한 이탈 주민 56명을 대상으로 4명의 훈련된 북한 이탈 주민 조사원들이 직접 대면하여 연구를 진행하였다. 인구학적 정보 및 탈북 관련 요인, 주관적 건강 인지, 정신건강 서비스 요구도에 대해 설문 조사하였고 벡 우울 척도와 벡 불안 척도를 사용하여 우울 및 불안 증상을 평가하였다. 결 과 : 대상자의 39.3%가 우울 증상을 호소하였고 42.9%가 불안 증상을 호소하였다. 우울군에서 무직인 경우(p=0.021), 정신건강 관련 질병을 가지고 있는 경우(p＜0.001), 북한에 직계 가족이 있을 경우(p=0.029)가 유의하게 많았으며, 불안군에서 주소득원이 정부 보조인 경우(p=0.001), 무직인 경우(p＜0.001), 기초생활 수급자인 경우(p=0.038), 의료 급여 1종 대상자인 경우(p=0.018), 정신건강 관련 질병을 가지고 있는 경우(p＜0.001), 주관적 건강 인지가 좋지 않은 경우(p=0.035)가 더 많았다. 그러나 탈북 동기(우울군 p=0.767, 불안군 p=0.727), 탈북 과정(우울군；체류 국가 수 p=0.448, 체류 기간 p=0.450, 불안군;체류 국가 수 p=0.268, 체류 기간 p=0.386), 현재 가족과의 동거(우울군 p=0.581, 불안군 p=0.139)는 유의한 차이를 보이지 않았다. 북한 이탈 주민들의 정신건강 서비스 요구도는 높았으며 상담 치료와 심리검사를 원하고 있었다. 결 론 : 북한 이탈 주민의 상당수에서 우울 또는 불안 증상이 나타났으며 과거 탈북 관련 요소보다는 경제적 상태와 주관적 건강 인지와 같은 현재 상태와 더 연관이 있는 것으로 나타났다. 정신건강 서비스 요구도 또한 선행 연구와 달리 높아 북한 이탈 주민의 우울 및불안에 초점을 맞춘 정신보건 서비스적 접근이 필요할 것으로 보인다.

영문 초록

Objective : Many North Korean refugees have been settled down in South Korea, and they have been suffering from pre-migration trauma and resettlement stress. And there are not enough systematic studies about general mental health of North Korean refugees. Our aim is to evaluate prevalence of depression and anxiety symptoms, to find factors affecting these symptoms, and to investigate mental health service needs for North Korean refugees. Methods : 56 subjects were recruited from North Korean adult refugees who lived in Gunpo-si. We trained 4 North Korean refugees as interviewers. The trained interviewers visited North Korean subjects and made the subjects to submit self- administered questionnaires about demographic factors, factors related to defection, subjective health perception, and mental health service needs. And the subjects were assessed with Beck depression inventory and Beck anxiety inventory to measure depressive and anxious symptoms, respectively. Chi-square test and independent T test were performed. Results : In 56 subjects, 39.3% of subjects showed depressive symptoms and 42.9% of them had anxiety symptoms. Unemployment (p=0.021), having mental illnesses (p<0.001) and lineal family members in North Korea (p=0.029) were more frequently found in depressed subjects. Also anxious subjects had having government grant as main income (p=0.001), unemployment (p<0.001), being recipient of national basic livelihood security (p=0.038), medical aid type 1 (p=0.018), bad perception of subjective health (p=0.035), and mental illnesses (p<0.001). But factors related to defection were not significantly associated with depression or anxiety symptoms. Many subjects reported to need mental health service and they want to get psychotherapy and psychological test. Conclusion : This study revealed that many North Korean refugees have depression or anxiety symptoms. And there were significant associations between symptoms and current status, such as financial problems and subjective health perception, rather than factors related to defection. And they wanted mental health services more than our expectation. Further evaluation about mental health services focusing on depression and anxiety symptoms is needed for North Korean refugees.
