정신의료기관 종사자들의 병원-지역사회 연계에 대한 인식과 실태에 관한 연구

국문 초록

목 적： 본 연구는 정신의료기관 종사자가 의료기관으로부터 퇴원하는 정신질환자를 지역사회정신보건기관으로 연계하는 것에 대해 어떻게 인식하고 경험하고 있는지 알아보고, 이를 시행하는데 장애가 되고 있는 요인을 탐색하고 개선하여 연계를 보다 활성화할수 있는 방안을 모색하고자 한다. 방 법： 서울 및 경기도 소재 병·의원 중 입원병상이 있는 44개 기관의 정신과전문의, 간호사, 사회복지사 258명을 대상으로 연계에 대한 인식 및 실태, 연계의 장애요인 등을 묻는 설문을 시행하고 수집된 자료를 분석하였다. 결 과： 병원의 유형이나 직종과 관계없이 병원 종사자들은 대부분 정신의료기관과 지역사회정신보건기관 사이의 연계체계 구축이 매우 필요하며 지역사회기관의 역할이 매우 중요하다는 것을 이해하고 있었다. 지역사회기관으로 연계를 시켜본 경험이 있는 사람은 61.6%였고, 평균 연계횟수는 8.17회(SD=10.266)이었다. 연계를 하면서 고려하는 정신보건서비스는 주간재활과 사례관리가 가장 주를 이루었으며, 이를 위해 정신보건센터에 가장 많은 대상자를 연계한 것으로 나타났다. 연계를 하는데 있어 장애의 요인으로는 지역사회정신보건기관에 대한 정보부족과 체계적인 연계 방법의 미비라는 응답이 가장 많았다. 결 론： 본 연구는 정신의료기관 종사자들은 대부분 의료기관으로부터 퇴원하는 정신질환자에 대하여 지속적인 정신보건서비스 제공을 위해 지역사회정신보건기관으로 연계를 하는 것에 대한 필요성을 느끼고 있지만, 지역사회기관에 대한 정보부족과 체계적인 연계경로가 갖추어지지 않아 연계가 효과적으로 이루어지지 못하고 있음을 보여주었다. 효과적인 연계를 위해서는 수요에 따른 공급의 확대를 통한 지역별 균형적이고 충분한 인프라구축이 전제되어야 하며, 지역사회정신보건기관에서는 다양한 교육 프로그 램의 개발, 특성화된 개별 서비스 제공, 홍보를 위한 노력을, 정신의료기관에서는 지역사회기관에 대한 정보를 얻고, 환자와 가족의 재활에 대한 동기를 극대화 할 수 있는 방법을 모색해야 할 것이다. 그리고 정책적으로 다양한 연계모형을 제시하고, 이를 수정, 보완해 가는 작업이 지속적으로 필요할 것이다.

영문 초록

Objectives：We investigated attitudes of professionals working in hospital toward linkage between hospital and community mental health facility for mentally ill people who are discharged from hospital, and also their experiences of referral to community facilities. So that we want to explore the obstacles against effective linkage, and consider how to manage those problems and make linkage system more active. Methods：Total 258 psychiatrists, nurses and social workers, working in mental hospital which had inpatient unit and are located in Seoul and Kyeonggi province were asked to fulfil our questionnaire including attitudes toward linkage, their experience, obstacles against linkage. Results：Most staffs in hospital consider the linkage system between hospital and community is essential and the role of mental health facilities in community is very important. Among the responders, 159(61.6%) staffs have referred clients to community facilities, and mean number of referral during last 1 year is 8.17 times(SD=10.266). They consider day care program, and case management service when they refer the patients to the community and for this purpose, they refer their patients to community mental health center the most. They consider lack of information on community services, and poorly organized referral system as two most serious obstacles against linkage. Conclusion：This study shows that staffs in hospital consider the linkage between hospital and community is so important and essential for continuing mental health services, but they have less information and their is no formal pathway and system of referral, so they have lots of difficulties when they perform and the linkage is not done effectively now. For making linkage system more effectively, most of all, sufficient and balanced infra-structure have to be constructed according to needs. And also, community staffs have to make efforts to develope education program, individual service plan, and public awareness. Staffs in hospital have to make efforts to get more information on community mental health services and related facilities, and find ways to how exaggerate the motivation of patients and care-givers for recovery and rehabilitation. Lastly, various linkage system models have to be suggested and examined, and we have to improve and modify those models.
