적정진료와 병동 설계 최적화

국문 초록

의사는 환자들에게 적정 수준의 진료를 제공하는 것을, 병원 경영자는 병원의 수익을 최대화하는 것을 목적으로 한다. 적정진료수준과 병원의 수익을 최대화하는 진료수준이 일치하지 않을 경우, 적정진료가 병원의 수익 최대화를 어렵게 만들수 있다. 본 연구는 적정진료시 효용을 높일 수 있는 병상 설계를 찾아 이러한 최적화 문제를 해결한다. 연구 결과, 적정진료수준과 병원의 효용을 최대화하는 진료수준이 불일치함을 확인하였다. 효용 최대화를 목적으로 할 경우, 병동이 세분화될수록 대부분의 진료과목에서 과잉진료가 발생하였다. 모든 진료과목에서 적정진료가 이루어질 경우, 총효용의 최댓값은 수익 최대화를 목적으로 했을 때의 최댓값에 도달하지 못하였다. 또한, 적정진료를 제공할 경우 병동 설계 방식에 따라 병원 효용의 편차가 매우 컸는데, 이는 병원 경영자가 효용을 최대화할 수 있는 최적의 병동 설계를 구상해내야 함을시사한다. 본 연구는 이와 같은 병원 경영자의 최적화 문제 해결에 단초를 제공한다.

영문 초록

Healthcare providers try to achieve an appropriate level of care, while administrators pursue maximization of hospital utilities. The appropriate level of care may make it hard to maximize the hospital’s utility. This study solves this problem by discovering a wing formation that maximizes the utility when the level of care is appropriate. Consequently, this study demonstrates that the appropriate level of care is different from the level which maximizes the hospital's utility. Specifically, as the wings are segmented, the overtreatment occurs in most care types. The value of maximum total utility is lower when the levels of all types of care are appropriate, compared to when the wings are formed to simply maximize the hospital’s utility. In addition, when the appropriate level of services is provided, the deviation of the hospital’s utility is high depending on the wing formations. Thus, when the appropriate level of care is provided, the hospital managers should identify an optimal wing formation while maximizing the utility. This study can provide a clue for solving such decision-making problems of hospital managers.
