국문 초록

본 연구는 건강보험 보장성 강화를 중심으로 추진되는 의료보장정책의 목표 달성이 어려운 현상에주목하고, 건강보험 보장성 향상 지체의 주요 원인이 의료보장제도가 갖는 정책문제의 복잡성에 있음을 살펴보았다. 본 연구에서는 의료보장체계를 제도복합체로 간주하고, 의료보장정책 문제의 복잡성차원을 수평적･수직적･시간적 차원으로 구분하였다. 수평적･수직적 차원의 복잡성은 의료보장정책을둘러싼 제도적 환경의 층화 구조와 제도적 논리의 다양성으로 파악하였다. 또한 시간적 차원의 복잡성은 제도적 논리를 중심으로 한 긍정적/부정적 정책환류가 동시에 작동함을 통해 살펴보았다. 분석결과, 한국 의료보장정책은 보건의료환경 및 복지체제의 변화, 의료거버넌스의 구성과 주요 행위자의 특성을 반영한 제도적 논리를 중심으로 진화하고 있는 것으로 나타났다. 의료보장정책의 제도적 논리인공공성과 시장성, 보장성과 효율성 측면에서 볼 때 공공성과 보장성을 확대･강화하는 정책을 추진하였지만, 시장성과 효율성의 논리가 함께 작동하면서 공공성과 보장성 확대･강화의 정책적 성과가 제약을받는 경향이 나타났다. 정책환류 측면에서 공공성과 보장성을 확대 강화하는 조치는 의료보장정책의긍정적 정책환류로, 시장성과 효율성 논리는 의료보장정책의 공공성과 보장성이 확대 강화되는 데 있어 부정적 정책환류로 작용하면서 의료보장정책의 변화와 연속이 나타나고 있음을 알 수 있다.

영문 초록

This study focuses on the difficulty in achieving the goal of health insurance coverage, which is the strengthening of health insurance coverage, and examines that the main cause of the delay in improving health insurance coverage lies in the complexity of the policy problem of the health care system. In this study, the health care system is regarded as an institutional complex, and the complexity of health care policy issues is divided into horizontal, vertical, and temporal dimensions. The complexity of the horizontal and vertical dimensions was identified as the stratified structure of the institutional environment surrounding medical security policy and the diversity of institutional logic. In addition, the complexity of the temporal dimension was examined through the simultaneous operation of positive/negative policy feedback centered on institutional logic. As a result of the analysis, it was found that Korea’s health care policy has evolved centering on institutional logic that reflects changes in the health care environment and welfare system, the composition of medical governance, and the characteristics of major actors. In terms of publicity and marketability, security and efficiency, which are the institutional logic of health care policy, policies to expand and strengthen publicity and security have been pursued. Also, there was a tendency to be constrained, too.
