갑상샘암종을 동반한 갑상샘눈병증의 임상 양상

국문 초록

목적: 갑상샘암종을 동반한 갑상샘눈병증 환자의 임상 양상에 대해 알아보고자 하였다. 대상과 방법: 갑상샘눈병증으로 본원에 내원한 환자 중 갑상샘암종을 동반한 33명의 환자를 대상으로 후향적 연구를 시행하였다. 안과 초진 및 갑상샘암종 진단 당시 갑상샘기능 상태, 갑상샘암종 종류, 갑상샘절제술 및 방사성요오드 치료 시행 여부, 임상활동점수, 안구돌출도, 눈꺼풀각막반사간거리1, 복시 여부, 안과 외래 방문 기간 및 안과적 치료 내용, 눈증상과 갑상샘암종 진단 및 갑상샘절제술의 선후관계에 대해 분석하였다. 결과: 전체 33명 환자의 평균 나이는 45.55세였다. 남성은 8명, 여성은 25명이었다. 전체 관찰 기간 중 임상활동점수가 가장 높을때의 방문 기준으로 평균 임상활동점수는 2.00 ± 1.67점, 안구돌출도의 평균은 19.28 ± 3.24 mm, 눈꺼풀각막반사간거리1의 평균은4.52 ± 1.58 mm로 측정되었다. 눈증상 발현이 갑상샘암종 진단 이전부터 있었던 환자는 갑상샘암종 진단 후 눈증상이 나타난 군에비해 갑상샘기능항진 경향을 보였다. 임상활동점수는 눈병증선행군에서 2.58 ± 1.77, 암종선행군에서 1.15 ± 1.07로 눈병증선행군에서 유의미하게 높았으며(p=0.016), 안구돌출도 역시 눈병증선행군에서 20.53 ± 3.20 mm, 암종선행군에서 17.46 ± 2.37 mm로 눈병증선행군에서 더 높았다(p=0.008). 안병증선행군에서 더 많은 안과적 치료를 요하는 경향이 있었다. 두 환자군 모두 갑상샘 자가항체수치의 평균이 정상에 비해 상승되어 있었다. 갑상샘눈병증선행군에서 갑상샘절제술 시행 후 안구돌출지수가 유의미하게 감소했다. 결론: 안과적 증상이 선행된 갑상샘암종 환자에서 갑상샘기능항진을 보일 수 있고 갑상샘암종선행군에 비해 심한 임상 양상을 보이며, 추가적인 안과적 치료가 필요할 수 있다. 또한, 갑상샘절제술 및 방사성요오드 치료 후에도 갑상샘눈병증이 새로이 발현될 수 있으므로, 이를 환자에게 수술 전 설명하고 수술 후 면밀히 관찰하는 것이 필요하겠다.

영문 초록

Purpose: To analyze the clinical manifestations of thyroid-associated ophthalmopathy accompanied by thyroid cancer. Methods: We performed a retrospective chart review of 33 patients diagnosed at our ophthalmology department with both thyroid- associated ophthalmopathy and thyroid cancer. We performed thyroid function tests at the first visit and at diagnosis of the thyroid cancer, and recorded the type of thyroid cancer, thyroidectomy history, clinical activity score (CAS), exophthalmometry, margin reflex distance (MRD) 1, diplopia, follow-up duration of ophthalmopathy, and ocular treatments. We divided patients into two groups according to the time of diagnosis of thyroid ophthalmopathy and thyroid cancer. Results: The average age of the 33 patients was 45.55 years, and eight and 25 were males and females, respectively. The average CAS score, exophthalmometry measurement, and MRD1 were 2.00 ± 1.67, 19.28 ± 3.24 mm, and 4.52 ± 1.58 mm, respectively. Patients whose ocular symptoms preceded the diagnosis of thyroid cancer tended to have a higher thyroid hormone level, CAS score, and exophthalmometry measurement, and required more ocular treatments, compared to the other patients. The average thyroid auto-antibody levels were increased in both groups. In patients whose ophthalmopathy preceded the thyroid cancer, exophthalmometry measurements were significantly decreased after thyroidectomy. Conclusions: It is crucial to identify ophthalmopathy in association with thyroid cancer. Patients whose ocular symptoms started before the diagnosis of thyroid cancer may present with hyperthyroidism and severe ocular symptoms, and may need ocular treatments. Patients should be informed that thyroid-associated ophthalmopathy may develop after thyroidectomy or radioiodine therapy, and their ocular symptoms should be closely monitored.
