국문 초록

오늘날 정신건강 문제는 흔한 질병이 되고 있으며 사회적 관심도 높아지고 있다. 이에 본 연구는 국내 119구급대원의 정신과적 응급환자 대응실태를 파악하여 개선방안을 제시하고자 했다. 119구급활동일지 분석과 119구급대원 대상 설문조사를 실시한 결과 119구급대원은 정신과적 응급환자를 대응하는 순간부터 병원에 도착하여 인계하는 모든 과정에서 어려움을 겪고 있었다. 이를 개선하기 위해서는 첫째, 응급위기개입서비스를 활성화하고 119구급대원과 경찰관의 교육과 훈련이 필요하다. 둘째, 자·타해 위험이 있는 정신과적 응급환자는 경찰의 구급차 동승 의무화와 119구급대원의 신체 구속을 법적으로 명시해 119구급대원의 안전을 확보해야 한다. 셋째, 정신과적 응급환자를 위한 유휴 병상 확보와 병원의 적극적인 협조가 필요하다. 넷째, 추가 손상이 없는 정신과적 응급환자의 단순 이송은 자가용 등의 다양한 이송수단을 활용한다. 마지막으로 지역 내 경찰, 병원, 소방 관계기관의 협력을 통해 위기사례를 공유하여 한국형 응급개입모델개발을 지속해야 한다.

영문 초록

The increase in mental health issues in recent years has increased social interest in mental illnesses. The purpose of this study is to identify the status of 119 paramedics practices regarding response to psychiatric emergency patients and suggest ways to improve them. For this study, Seoul s 119 first aid log in 2019 were analyzed, along with a survey of 119 paramedics working in Seoul. The findings show that the 119 paramedics surveyed experience difficulties throughout the entire process, from their first encounter with psychiatric emergency patients to their arrival at, and transfer to, the hospitals. Improvements on these issues require efforts in five areas. First, emergency crisis intervention service should be reinvigorated. In addition, 119 paramedics and police officers need regular training on how to handle psychiatric patients. Second, when transporting a psychiatric emergency patients who may harm themselves or others, the mandatory accompaniment of police officers and the restraint of paramedics should be specified in the law. Third, hospitals needs to provide full cooperation and secure hospital beds for psychiatric emergency patients. Fourth, in the case of transferring psychiatric patients without severe damage should be carried out with modes of transportation other than the ambulance, including private cars. Finally, the police, hospitals and firefighting agencies within each region should work together to share their experience with crisis response, thereby continuing the efforts to develop Korean-style emergency crisis intervention models.
