소변 유기산 검사 결과에 따른 맞춤 영양 치료

국문 초록

소변 유기산 검사는 각 대사 경로에서 만들어지는 대사 산물들이 체 내에 축적되고 소변으로 배출되는 것을 검출하는 정밀한 검사 방법이다. 과거 영유아 선천성 대사 이상을 찾기 위해 주로 사용되었지만 최근에는 성인에서의 대사 불균형을 찾기 위한 기능의학적 해석으로도 활용하고 있다. 본 종설에서는 소변 유기산 검사에서의 대표적인 소견들과 그에 따른 영양 치료에 대해 살펴보도록 한다. 에너지 대사와 관련한 다양한 지표들은 주로 지방산, 탄수화물 대사와 구연산회로를 중심으로 한 에너지 대사 산물들을 표시하고 있다. 지방산 대사의 베타산화 이상 지표들이 상승되면 카르니틴을 중심으로 영양치료를 시행할 수 있고, 탄수화물 대사 산물들이 상승하면 비타민 B군과 리포산, coenzyme Q10 등을 통한 조효소 활성화를 권장할 수 있다. 구연산회로는 자체적 회로와 각종 아미노산 대사로부터 편입되는 물질들이 복합적으로 작용하므로 각 효소 활성도와 아미노산 대사를 동시에 고려해야 한다. 또, 일부 아미노산의 대사 과정에서 엽산, 비타민 B6, B12, 비오틴 등의 부족으로 인한 특이 지표 상승이 보인다면 해당 영양치료를 시행할 수 있다. 이와 같이 소변 유기산 검사를 통해 다양한 대사 과정 불균형에 대한 정보를 종합하고 환자의 기능의학적 치료방향을 수립하는데 도움을 받을 수 있을 것이다.

영문 초록

Urine organic acid profiles are one of the finest methods to detect metabolites produced by various metabolic pathways. While this method has been used mainly for inborn errors of infants previously, now it is also widely used to detect the metabolic imbalances in adults. This review showed various findings in urine organic acid profiles and the nutritional therapy according to the results. The indices of energy metabolism consist of metabolism of fatty acid, carbohydrate, and citric acid cycle. Generally, high levels of the intermediate metabolites derived from beta-oxidation of fatty acids indicate the requirement of nutritional therapy including carnitine. Imbalances in carbohydrate metabolism may indicate the requirement to activate the cofactors including vitamin B, lipoic acid, and coenzyme Q10. Since citric acid cycle covers both each intermediate metabolite and materials from amino acid metabolism, nutritional therapy should include both cofactors for the cycle and amino acid metabolism. Specific metabolites of amino acids produced by deficiencies in specific vitamins may indicate the need for the supplementation. In summary, urine organic acid profiles provide information on the imbalances of metabolic pathways and give physicians opportunity to make plans of functional approaches for patients.
