눈물길미세내시경 소견에 따른 실리콘관삽입술의 성공률 분석

국문 초록

목적: 눈물길폐쇄 환자에서 눈물길미세내시경상 관찰되는 폐쇄의 위치와 정도가 실리콘관삽입술의 성공률에 미치는 영향을 평가하고자 하였다. 대상과 방법: 2018년 8월부터 2019년 10월까지 눈물길미세내시경하 실리콘관삽입술을 시행 받은 120안 74명의 환자들을 대상으로 후향적 차트 리뷰를 시행하였다. 수술 성공률과 환자의 임상양상, 수술 전 관류검사의 결과, 눈물길미세내시경 관찰양상을 각각 비교 분석하였으며, 눈물길미세내시경상 관찰되는 폐쇄의 위치와 정도를 분류하였다. 결과: 눈물길미세내시경하 실리콘관삽입술을 시행 받은 120안 중 수술 성공은 101안(84.2%)이었다. 미세내시경상 눈물소관협착 41안(34.2%), 눈물소관폐쇄 35안(29.2%), 코눈물관협착 24안(20.0%), 코눈물관폐쇄 28안(23.3%)이 관찰되었다. 눈물소관과 코눈물관의폐쇄의 위치에 따라 성공률에는 차이가 없었으며, 폐쇄 정도에 따라 성공률에 영향을 미치는 것을 확인하였다(p=0.041, p=0.018). 결론: 눈물길미세내시경 소견을 바탕으로 코눈물관폐쇄를 분류할 수 있으며, 폐쇄의 위치는 실리콘관삽입술의 성공률과 관계가 없었고, 눈물길폐쇄의 정도는 실리콘관삽입술의 성공률에 통계적으로 유의한 영향을 미침을 알 수 있었다.

영문 초록

Purpose: In patients with lacrimal passage obstruction, the location and severity of the obstruction observed in lacrimal endoscopy were evaluated with respect to the success rate of silicone tube intubation. Methods: We conducted a retrospective chart review of 120 eyes of 74 patients who underwent lacrimal endoscopy (RUIDO fiberscope; Fibertech Co., Tokyo, Japan)-assisted silicone tube intubation from August 2018 to October 2019. The analysis included clinical characteristics, irrigation test results, lacrimal endoscopic findings, and analyzed factors associated with surgical success. The location and severity of the obstruction observed by lacrimal endoscopy were classified. Results: Of the 120 eyes that received lacrimal endoscopy-assisted silicone tube intubation, 101 eyes (84.2%) were successful in surgery. Canalicular stenosis in 41 eyes (34.2%), canalicular obstruction in 35 eyes (29.2%), nasolacrimal duct stenosis in 24 eyes (20.0%), and nasolacrimal obstruction in 28 eyes (23.3%) were observed in lacrimal endoscopy. Regardless of the location of the obstruction of the canaliculus and nasolacrimal duct, it was observed that surgical success was affected by the severity of the obstruction (p = 0.041 and p = 0.018, respectively). Conclusions: The severity of the lacrimal passage obstruction observed during lacrimal endoscopy was shown to have a statistically significant effect on the success rate of lacrimal endoscopy-assisted silicone tube intubation. However, the location of the obstruction in the lacrimal passage did not have a statistically significant effect on the success rate.
