일차 열공망막박리 환자에서 맥락막두께에 대한 분석

국문 초록

목적: 단안의 일차 열공망막박리 환자에서 망막박리안의 중심오목하 및 망막박리 부위의 맥락막두께를 정상안과 비교 분석하고자 하였다. 대상과 방법: 단안의 일차 열공망막박리로 2013년 1월부터 2016년 12월까지 수술 받은 환자 중 초진시 양안의 빛간섭단층촬영을 시행한 환자를 후향적으로 분석하였다. 맥락막두께는 독립적인 2명의 안과 의사에 의해 각각 수동으로 분석되었으며, 중심오목하 맥락막두께와 망막박리 부위의 맥락막두께를 측정하였다. 결과: 총 95명의 환자가 대상으로 포함되었다. 망막박리가 황반부를 침범한 경우는 61명(64.2%), 중심오목을 침범한 경우는 44명(46.3%)이었다. 전체 환자에서 망막박리안의 중심오목하 맥락막두께는 244.7 ± 79.0 μm로 정상안 해당 영역의 220.1 ± 78.9 μm보다 통계적으로 유의하게 두꺼웠다(p<0.001). 중심오목박리가 있는 경우, 망막박리안에서 정상안보다 중심오목하 맥락막두께가 통계적으로 유의하게 두껍게 측정되었다. 또한 망막박리 부위의 맥락막두께는 273.8 ± 53.4 μm로 망막이 박리되지 않은 부위(215.2 ± 44.0 μm)보다 유의하게 두꺼웠으며(p<0.001), 정상안의 대칭되는 부위(233.3 ± 56.7 μm)와 비교해서도 유의하게 두꺼워짐을 확인하였다(p<0.001). 결론: 일차 열공망막박리 환자에서 망막박리 부위의 맥락막두께는 정상안의 동일 부위에 비해 두꺼워져 있음을 확인하였다. 추후 맥락막의 변화 기전과 망막박리에 미치는 영향에 대한 추가적인 연구가 필요할 것으로 사료된다.

영문 초록

Purpose: We analyzed the choroidal thickness in the subfoveal area and area of retinal detachment (RD) in monocular primary rhegmatogenous retinal detachment (RRD) patients and compared the RD eye with the fellow eye. Methods: We retrospectively analyzed, optical coherence tomography data of both eyes at the first visit in patients who underwent surgery for monocular RRD from January 2013 to December 2016. Choroidal thickness was examined manually in the subfoveal and RD areas by two independent ophthalmologists; subjects without data from at least one of the two investigators were excluded. Results: Ninety-five subjects were included in this study. Of the 95 RD eyes, 61 (64.2%) and 44 (46.3%) eyes showed macula and fovea invasion, respectively. The subfoveal choroidal thickness was statistically significant in the RD eye compared with the fellow eye (244.7 ± 79.0 μm vs. 220.1 ± 78.9 μm; p < 0.001). Moreover, the choroidal thickness of the RD area in the RD eye was thicker compared with the non-detachment area and corresponding area in the fellow eye (273.8 ± 53.4 μm vs. 215.2 ± 44.0 μm and 233.3 ± 56.7 μm, respectively; p < 0.001). Conclusions: In the primary RRD eye, the choroidal thickness in the detachment area was thicker than that of the fellow eye. Further studies are needed to investigate the mechanism of choroidal thickness change and its impact on RD.
