의학적으로 설명되지 않는 증상에 대한 기능의학적 접근과 관리

국문 초록

의사들은 일차 의료를 비롯한 많은 임상 상황에서 의학적으로 설명되지 않는 다양한 증상들을 호소하는 환자들을 자주 접하게 되며, 이것을 ‘의학적으로 설명되지 않는 증상들(medically unexplained symptoms, MUS)’이라고 부른다. 정신의학에서는 이러한 MUS 환자들을 보통 신체 증상 장애(somatic symptom disorder)로 분류하나, 모든 MUS 환자들을 이러한 정신과적 문제로 완벽하게 설명할 수 없으며 아직 신체 증상 장애에 대한 효과적인 치료나 관리법도 확인되지 않은 상태이다. 다행히 최근 발전중인 기능의학(functional medicine)은 기존의 의학적 검사 방법으로 진단되지 않는 인체의 기능적 이상을 전반적이고 자세한 병력 청취(retelling the patient’s story, core clinical imbalance 파악 등) 및 신체 진찰과 함께 모발 미네랄 검사, 소변 유기산 검사 등의 최신 검사 방법을 활용해 찾아내고, 이를 영양 치료나 운동 치료, 식이 요법 등 다양한 방법을 통해 치료, 관리하여 환자가 최적의 건강 상태를 유지할 수 있도록 해주는 통합적인 학문으로, MUS 환자들에게 기존의 의학적 접근보다 더 좋은 임상적 효과와 환자 만족을 달성할 수 있을 것으로 보이며 앞으로 이 분야에 대한 지속적인 관심과 연구가 필요할 것으로 생각된다.

영문 초록

Medically unexplained symptoms (MUS) are very common, especially in primary care area. MUS are important because they can cause similar levels of disability and social burden as symptoms caused by disease. In the diagnostic and statistical manual of mental disorders-fifth edition, these patients are usually classified ‘somatic symptom disorder’, but there is no definite treatment option for this. However, functional medicine, the recently developed medical area focusing on optimal functions and core clinical imbalances of human body, can provide better explanations and management plans for MUS than conventional medicine (such as psychiatry or internal medicine). In functional medical approach, doctors should take complete medical history and perform physical examination especially focusing on environmental factors, personal lifestyle, genetic predisposition, triggers and core clinical imbalances. New precise laboratory techniques like hair tissue mineral analysis or urinary organic acid profile can give important functional data. Then, functional medical doctors can manage MUS with intensive lifestyle modification and personalized nutrition therapy based on their core clinical imbalances. In this paper, the author reviews the up-to-date principles and practical methods of functional medical approach and management for MUS.
