녹내장수술 후 안압하강에 따른 맥락막두께의 장기 변화

국문 초록

목적: 녹내장수술 후 안압하강에 따른 안축장, 안구관류압 및 맥락막두께의 장기 변화를 분석하고, 맥락막두께 변화에 영향을 미치는 인자를 알아보고자 하였다. 대상과 방법: 원발개방각녹내장환자 71명 71안을 대상으로 녹내장수술 후 1년동안 전향적 연구를 시행하였다. 수술 전, 수술 후 1주일, 1개월, 2개월, 6개월, 1년째 스펙트럼영역빛간섭단층촬영기를 이용하여 중심와아래와 시신경유두주위 맥락막두께를 측정하였다. 안압 및 혈압을 측정하여 안구관류압을 계산하였고 부분결합간섭계를 이용하여 안축장을 측정하였으며 이들 간의 상관관계를 분석하였다. 결과: 수술 후 1년간 안압의 하강과 함께 안축장은 유의하게 감소하였고, 안구관류압, 중심와아래 및 시신경유두주위 맥락막두께는 유의하게 증가하였다(모두 p<0.001). 중심와아래 및 시신경유두주위 맥락막두께 변화는 안압의 변화와 유의한 상관관계가 있었다(r=-0.519, r=-0.528). 특히, 안압이 30% 이상 하강한 환자에서 중심와아래 및 시신경유두주위 맥락막두께가 그렇지 않은 환자보다 더 많이 증가하였고(p=0.001, p=0.002), 안축장이 24 mm 이하인 환자에서 24 mm를 초과하는 환자보다 중심와아래 맥락막두께가 유의하게 많이 증가하였다(p=0.044). 결론: 녹내장수술 후 안압의 하강에 따른 안축장 감소, 안구관류압의 증가, 그리고 맥락막두께의 증가는 1년간 경과 관찰기간 동안 지속되었다. 이는 녹내장수술이 안압하강으로 시신경섬유에 대한 기계적 압박을 감소시키고 안혈류를 증가시킬 수 있을 것으로 추측된다.

영문 초록

Purpose: We investigated the long-term longitudinal changes in axial length (AL), mean ocular perfusion pressure (MOPP), and choroidal thickness (CT) according to the reduction of intraocular pressure (IOP) after glaucoma surgery. The potential variables associated with CT changes were also evaluated. Methods: This was a prospective study for 1 year after glaucoma surgery, which included 71 eyes of 71 patients with primary open-angle glaucoma. The subfoveal CT (SFCT) and peripapillary CT (PPCT) were measured using spectral-domain optical coherence tomography preoperatively and 1 week, 1 month, 2 months, 6 months, and 1 year postoperatively. MOPP was calculated from the IOP and blood pressure. The AL was measured using partial coherence interferometry. Regression analysis was conducted to assess the possible association of variables. Results: The AL decreased and the MOPP, SFCT, and PPCT increased significantly with IOP reduction at 1 year postoperatively (all, p < 0.001). The changes in SFCT and PPCT were significantly associated with IOP reduction at 1 year postoperatively (r = -0.519 and r = -0.528, respectively). Importantly, greater increases in SFCT and PPCT were found in patients with IOP reduction more than 30% from baseline, when compared with those with less than 30% reduction (p = 0.001 and p = 0.002, respectively). The SFCT increased more significantly in patients with AL ≤ 24 mm, compared with patients with AL > 24 mm (p = 0.044). Conclusions: Reduction in the IOP, increase in the MOPP, decrease in the AL, and increase in the CT after glaucoma surgery persisted for 1 year during a long-term follow-up. These results suggested that glaucoma surgery reduced mechanical compression on the optic nerve fiber and increased intraocular blood flow.
