재가 노인과 시설 노인의 장기요양 돌봄 비용에 관한 비교 연구

국문 초록

본 연구는 재가 노인과 시설 노인의 장기요양 돌봄 비용을 비교 분석하여 장기요양 돌봄 비용의 효율적인 사용에 대한 기초자료를 제공하는 데 그 목적이 있다. 본 연구의 재가 노인과 시설 노인의 장기요양 비용을 비교 분석한 결과는 두 집단의 비용 차이가 있는 것으로 나타났다. 첫째, 재가 노인의 1인당 요보호 평균 총비용이 시설 노인의 1인당 요보호 평균 총비용보다 더 높은 것으로 나타났다. 둘째, 재가 노인의 생활비는 시설 노인의 생활비보다 더 낮은 것으로 나타났다. 셋째, 재가 노인의 요양인건비가 시설 노인의 요양인건비보다 더높은 것으로 나타났다. 넷째, 장기요양수급 노인의 특성에 따른 차이 분석에서도 성별, 연령, 장기요양등급, 질병의 수에 따라 재가 노인의 1인당 요보호 비용이 시설 노인의 비용보다 더 높게 나타났다. 하지만, 사회적 측면에서 두 집단의 직접비용만을 비교한 결과에서는 시설 노인의 1인당 요보호비용이 재가 노인의 비용보다더 높은 것으로 나타났다. 이러한 비용의 차이는 간접비용의 요인으로 무급의 가족에 의한 돌봄 비용으로 추정되었다. 가족에 의한 돌봄 비용은 약 58만 원으로 추정되었고 이는 장기요양서비스를 이용하고 있음에도 하루 평균 3시간 보호 비용이 더 필요하다는 것을 보여주고 있다. 이러한 결과는 사회적 관점에서 재가 노인의 가족에 의한 돌봄 비용이 장기요양재정을 절감하는 요인임을 보여주고 있다. 따라서 본 연구 결과는 장기 요양재정지출에 관한 정책 결정 시에 직접비용뿐만 아니라 가족에 의한 돌봄 비용까지 고려되어야 함을 시사 하며 가족에 의한 돌봄 서비스의 제도적 보상을 위한 기초자료로 활용될 수 있기를 바란다.

영문 초록

The purpose of this study is to provide basic data on the efficient use of long-term care costs by comparing and analyzing the long-term care costs of the elderly at home and in the facilities. The result of comparative analysis of the long-term care costs of the elderly at home and the elderly in the facility in this study showed that there was a difference in the costs of the two groups. First, It was turned out ** that the average total cost per person for the elderly at home is higher than the average total cost for the facility s elderly. Second, This shows the living cost of the elderly at home is higher than that of the elderly in facilities. Third, personnel expense for care of the elderly at home was found to be higher than that of the elderly in the facilities. Also, in the analysis of differences based on the characteristics of the elderly in long-term care, the cost of protection per person for the elderly at home was higher than that of the elderly in the facility, depending on gender, age, long-term care grades and the number of diseases. However, protection cost per person of the elderly in the facilities was found to be higher than that of the elderly at home in the result of comparing direct cost of the two groups in social aspect. The difference in these costs was estimated as the cost of caring by unpaid families labor as a factor in indirect costs. The care cost by family was estimated at 580,000 won and it showed that the cost of protection for 3 hours a day averagely is more needed, even though the long-term care service is used. Also from a social perspective, it shows that the cost of caring by the family of elderly at home is a factor reducing the long term care budget. Therefore, the results of this study suggest that not only direct costs but also family carer costs should be taken into account when making policy decisions on long term care spending, and I hope this can be used as basic data for institutional compensation of care services by family members.
