백내장수술 시 시행한 고전 단안시 요법 후 우세안 변화에 대한 임상 연구

목적: 고전 단안시 요법으로 양안 백내장수술을 받은 환자에서 우세안의 변화 여부를 알아보고 우세안의 변화에 영향을 미치는 요소를 알아보고자 한다. 대상과 방법: 양안 백내장수술을 시행받은 환자 중 고전 단안시 요법으로 수술을 시행한 70명의 환자를 대상으로 후향적 연구를 시행 하였다. 수술 전후 우세안이 변화한 환자군과 변하지 않은 환자군으로 나누어 수술 전후의 원거리 및 근거리 나안시력, 최대교정시력, 구면렌즈대응치, 입체시, 양안 수술 간의 시간 간격 등을 조사하였다. 결과: 연구에 포함된 총 70명의 환자에서 평균 나이는 71.26 ± 10.84세(범위 25-90), 평균 수술 간격은 118.46 ± 183.50일(범위 17-1,018), 양안의 수술 후 도수 차이는 1.16 ± 0.53디옵터(범위 0.00-2.75)였다. 수술 후 우세안이 변한 환자군이 22명(31.43%), 우세 안이 변하지 않은 환자군이 48명(68.57%)이었으며, 두 군 간의 양안 수술 간격, 수술 전 나안시력, 수술 전후의 최대교정시력, 입체시 등은 두 군 간에 큰 차이가 없었다. 우세안의 변화가 있는 환자군에서는 변화가 없는 환자군에 비하여 수술 전 양안의 구면렌즈대응치 차이가 더 작았고, 이는 통계적으로 유의하였다(t-test, p<0.05). 결론: 고전 단안시 요법으로 백내장수술을 한 환자에서 수술 후 우세안이 변하여 근거리 시력을 목표로 한 눈이 우세안이 된 환자는 22명(31.43%)이었으며, 우세안이 변화된 군에서는 우세안이 변하지 않은 군보다 수술 전 양안의 구면렌즈 대응치의 차이가 작은 경향을 보였다.

영문 초록

Purpose: The purpose of this study was to evaluate whether eye dominance changes after conventional pseudophakic monovision, and to identify factors that affect changes in eye dominance. Methods: This retrospective study included 70 patients who underwent bilateral conventional monovision cataract surgery. Patients were divided into two groups based on whether they experienced a change in the dominant eye. We compared patients’ uncorrected distance visual acuity (UCDVA), uncorrected near visual acuity (UCNVA), best-corrected visual acuity (BCVA), spherical equivalent, stereopsis, and time interval between cataract surgeries. Results: The mean age was 71.26 ± 10.84 (range, 25-90) years, mean interval between surgery in each eye was 118.46 ± 183.50 (range, 17-1,018) days, and mean postoperative diopter difference was 1.16 ± 0.53 (range, 0.00-2.75) diopters. After bilateral cataract surgery, 22 patients (31.43%) experienced a change in eye dominance, whereas 48 patients (68.57%) experienced no change. There were no differences in the time interval between cataract surgeries, preoperative UCDVA and UCNVA, pre- and postoperative BCVA, or stereopsis in either group. Patients who experienced a change in eye dominance showed smaller differences between preoperative and postoperative spherical equivalent, compared with patients who experienced no change in eye dominance (t-test, p < 0.05). Conclusions: Twenty-two (31.43%) patients whose nondominant eyes were targeted for near vision showed altered eye dominance after conventional monovision cataract surgery. Eye dominance shows greater plasticity in patients with smaller differences between preoperative and postoperative spherical equivalent. J Korean Ophthalmol Soc 2019;60(6):534-540
