한국인에서 의안착용 후 선호하는 의안의 방향과 눈꺼풀의 높이

국문 초록

목적: 한국인 의안환자에서 의안의 방향과 위눈꺼풀의 높이에 따른 선호도를 평가하고자 하였다. 대상과 방법: 2016년 8월에서 2018년 3월까지 의안을 착용한 환자 46명 46안을 대상으로 하였다. 정면 얼굴사진을 촬영한 후 Photoshop 프로그램을 이용하여 좌우 반전시켜 원본 사진과 좌우 반전한 사진 중 선호하는 것을 고르게 하고, 의안의 위눈꺼풀의 높이를 1 mm 상향, 1 mm 하향 조정한 후 환자에게 원본, 상향, 하향 사진 중 선호하는 것을 고르게 하였다. 결과: 우안(23안), 좌안(23안)이었으며 좌우 반전을 선호하는 환자는 22명(47.8%), 원본을 선호하는 환자는 24명(52.2%)이었다. 또한 좌안이 의안인 사진을 선호하는 환자는 31명(67.4%)으로 우안이 의안인 사진을 선호하는 15명(32.6%)보다 유의하게 많았다(p=0.02). 위눈꺼풀의 높이별로는 원본 선호자가 6명(13.0%), 1 mm 상향 선호자가 30명(69.3%), 1 mm 하향 선호자가 10명(21.7%)이었다(p<0.01). 양안 눈꺼풀 각막반사간거리1 (MRD1)의 차이가 1 mm 이하의 군을 A군, 의안의 MRD1이 정상안의 MRD1보다 1 mm 이상 작은 군을 B군, 의안의 MRD1이 정상안의 MRD1보다 1 mm 이상 큰 군을 C군으로 나누어 시행한 조사에서는 A와 B군에서는 1 mm 상향을, C군에서는 원본을 선호하였다(p<0.01). 결론: 의안 환자들은 의안의 위눈꺼풀 높이가 정상안에 비해 1 mm 이상 낮거나 1 mm 이내 차이가 있을 때 의안의 위눈꺼풀을 1 mm 올린 사진을 유의하게 선호하였다. 또한 의안의 원방향과 상관없이 좌안이 의안인 사진을 선호하였다. <대한안과학회지 2019;60(4):308-314>

영문 초록

Purpose: To evaluate the preferred eyelid height and laterality according to differences in marginal reflex distance 1 (MRD1) in Korean patients with an ocular prosthesis. Methods: A study of 46 eyes was conducted for patients with an ocular prosthesis between August 2016 and March 2018. In a slide show with the original and mirror-reversed photographs and upper eyelid height 1 mm above and below the base level, patients were asked which of the photographs of themselves they preferred. Results: Patients divided evenly into 23 individuals with a right side ocular prosthesis and 23 individuals with a left side ocular prosthesis. Among the patients, 47.8% preferred the mirror-reversed photograph and 52.2% preferred the original photograph. Meanwhile, 67.4% with the left side ocular prosthesis preferred the original photograph and 32.6% with the right side ocular prosthesis preferred the original photograph (p = 0.02). Additionally, 69.3% preferred their upper eyelid height 1 mm above the original photograph, 21.7% preferred their upper eyelid height 1 mm below the original photograph, and 13.0% preferred the level of the original photograph (p < 0.01). Twenty-five patients had < 1 mm difference of MRD1 (group A). Sixteen patients had 1 mm smaller MRD1 with an ocular prosthesis than with a normal eye (group B). Five patients had 1 mm larger MRD1 with an ocular prosthesis than with a normal eye (group C). Group A and B patients preferred the photograph with their upper eyelid 1 mm above from the original photograph (p < 0.01). Conclusions: Patients with an ocular prosthesis significantly preferred the photograph of their upper eyelid height 1 mm above the base level when the height of the ocular prosthesis was smaller or similar to the normal eye. And patients preferred the photographs with left sided prosthesis regardless of their original direction. J Korean Ophthalmol Soc 2019;60(4):308-314
