눈꺼풀 황색판종의 임상 양상 및 국소피판술을 이용한 위눈꺼풀성형술의 임상적 효과

국문 초록

목적: 눈꺼풀 황색판종 환자의 임상 양상 및 국소피판술을 이용한 위눈꺼풀성형술의 임상적 효과를 알아보고자 한다. 대상과 방법: 2010년 1월부터 2017년 12월까지 눈꺼풀 황색판종으로 내원한 환자 12명의 의무기록 및 수술 전후 사진을 후향적으로 비교 분석하였다. 모든 환자들에서 국소마취하 황색판종 제거 및 국소피판술을 이용한 위눈꺼풀성형술이 시행되었다. 결과: 12명(18눈꺼풀)을 대상으로 하였으며, 환자의 평균 나이는 47.0세였다. 남자와 여자는 각각 3명(25.0%)과 9명(75.0%)이었다. 눈꺼풀 황색판종이 양쪽에 있는 환자는 7명(58.3%), 한쪽에 있는 환자는 5명(41.7%)이었다. 검사상 4명(33.3%)에서 고지혈증이 있었 으며, 2명(18.2%)은 고중성지방혈증(hyper-triglyceridemia), 2명(18.2%)은 고저밀도지질단백혈증(hyper-low density lipoproteinemia) 이었다. 2명은 재발된 경우였으며, 동일하게 수술을 시행하였다. 수술 합병증은 없었으며, 술 후 단안에서 비대칭을 호소한 경우 이외 에는 모두 만족하였으며, 전체 환자 만족도는 83.3% (10명/12명)였다. 결론: 눈꺼풀 황색판종은 주로 중년 이상에서 양측 위눈꺼풀에 발병하는 경우가 흔하다. 눈꺼풀 황색판종의 치료로 국소피판술을 이용한 위눈꺼풀성형술은 환자의 만족도가 높은 치료법이며, 특히 안검하수 또는 눈꺼풀이완증을 동반하는 경우에 더욱 효과적이다. 다만 한쪽만 수술을 하는 경우 술 후 눈꺼풀 모양의 비대칭이 발생할 수 있음을 염두에 두어야 한다. <대한안과학회지 2018;59(9):803-809>

영문 초록

Purpose: To evaluate the clinical characteristics of xanthelasma palpebrarum as well as surgical results of upper blepharoplasty with a local flap technique. Methods: We conducted a retrospective cross-sectional study including 12 patients who underwent removal of xanthelasma palpebrarum and upper blepharoplasty with the local flap technique from January 2010 to December 2017. Results: A total of 18 eyelids of 12 patients were included in this study. The average age was 47.0 years and three patients (25.0%) were male and the other nine patients (75.0%) were female. There were seven patients (58.3%) on both sides of xanthelasma palpebrarum, and five patients (41.7%) on one side. There was hyperlipidemia in four patients (33.3%), hypertriglyceridemia in two patients (18.2%), and hyper-low density lipoproteinemia in two patients (18.2%). Two patients had recurrence and the same surgery was performed. There was no postoperative complication. All patients were satisfied except for asymmetry in the postoperative end, and overall postoperative satisfaction was 83.3% (10/12 patients). Conclusions: Xanthelasma palpebrarum often develops in both upper eyelids in middle-aged or older patients. Upper blepharoplasty with a local flap technique for the treatment of xanthelasma palpebrarum is an effective treatment for patients, especially in cases of ptosis or blepharochalasis. However, it should be noted that asymmetry of eyelids may occur after the surgery when surgery is performed on only one side. J Korean Ophthalmol Soc 2018;59(9):803-809
