미국백내장굴절수술학회 제공 공식을 포함한 굴절교정수술 후백내장수술 환자의 인공수정체 도수 계산 방법 비교

국문 초록

목적: 근시 각막 굴절교정수술 시행 후 백내장수술을 받은 환자들을 분석하여 굴절교정수술 후 인공수정체 도수 계산 공식들을 비교해 보고자 하였다. 대상과 방법: 총 20명 23안을 대상으로 American Society of Cataract and Refractive Surgery (ASCRS) Post-Refractive intraocular lens (IOL) Calculator 중 Haigis-L, Barrett no history 공식과 Shammas no history method, IOLMaster 700의 Haigis suite, clinical history method로 계산된 인공수정체 예측 오차를 비교하였다. 결과: ASCRS의 Barrett no history method가 가장 작은 산술 오차(-0.38 ± 0.99 D)를 보였으며, 절대 오차의 경우 ASCRS의 Haigis-L이 가장 작았다(0.76 ± 0.68 D). 술 전 각 방식으로 계산한 굴절값과 술 후 실제 환자의 굴절값 차이를 비교해 보았을 때 ASCRS의 Barrett no history method에서 ±0.50 D 이내가 52.2%, ±1.00 D 이내가 82.6%였다. 결론: 본 연구에서 근시 굴절교정수술 시행 후 백내장수술 시 인공수정체 도수 예측에서 ASCRS calculator의 Barrett no history 방식과 Haigis-L 방식이 적은 산술 오차 및 절대 오차를 보였다. ASCRS의 Barrett no history 방식은 다른 공식들에 비하여 술 후 0.50 D 이내 및 1.00 D 이내의 차이를 보이는 비율이 높았다. <대한안과학회지 2018;59(9):827-833>

영문 초록

Purpose: Methods of intraocular lens (IOL) calculations for cataract surgery after myopic corneal refractive surgery were compared. Methods: The medical records of 23 eyes from 20 patients were retrospectively analyzed. Five methods - Haigis L and Barrett no history method from Post-Refractive IOL Calculator of American Society of Cataract and Refractive Surgery (ASCRS), Shammas no history method, Haigis suite from IOL Master 700, and clinical history method - were used. Results: The Barrette no history method from the ASCRS showed the lowest arithmetic errors, while the Haigis-L from the ASCRS showed the lowest absolute prediction errors (-0.38 ± 0.99 diopters [D] and 0.76 ± 0.68 D, respectively). The percentages of refractive prediction errors within 0.50 D and 1.00 D of the Barrett no history method were 52.2% and 82.6%, respectively. Conclusions: The Barrette no history method and Haigis-L from the ASCRS showed the smallest errors in this study. The Barrett no history method showed a high percentage of refractive prediction errors within 0.50 D and 1.00 D. J Korean Ophthalmol Soc 2018;59(9):827-833
