MDCT의 정량적 분석 Software를 통한 Mandible의 Bone Mineral Density에 대한 고찰

국문 초록

목적 Mandible QCT검사의 T-Score값과 DEXA검사의 T-Score값을 비교하여 두 검사간의 유의성과 QCT의 Sensitivity를 알아보고자 하였다. 대상 및 방법 2010년 3월 2일부터 2010년 5월 4일까지 임플란트시술을 위해 본원을 내원한 환자 중 DEXA검사와 L-Spine QCT검사를 모두 시행한 환자 20명을 대상으로 하였다. 대상환자의 평균연령은 70.7세이며, 남성은 5명, 여성은 15명이었다. QCT와 DEXA검사간의 유의성을 비교하기 위해, 각각의 검사에서 평균 T-Score값을 측정하여 비교 분석하였다. QCT의 T-Score값의 Analize장치로는 Extended Workspace(Version 3.5, Philips Medical System, Netherlands)를 사용하였고, DEXA의 T-Score값은 Lunar Dual Energy X-Ray Bone Densitometa(Prodigy Advance, GE Health Care)를 사용하였다. 통계도구는 SPSS(Version 12.0 For Window)를 이용하였다. 결과 Mandible QCT와 L-Spine DEXA, Total DEXA의 평균 T-Score값을 그룹간 ANOVA 분석한 결과 P-value 0.001로 유의한 차이가 있었으며, 이를 대응 표본 T-test를 total DEXA와 L-Spine DEXA간에 한 결과 P-value 0.033, total DAX와 Mandible QCT간에 한 결과 P-value 0.003, L-spine DEXA와 Mandible QCT간에 한 결과 P-value 0.001로 나와 각각의 그룹간에도 유의한 차이를 보였다. 결론 Mandible에 적용된 QCT의 Bone Mineral Density DATA값들이 DB(Data base)로 구축되어 진다면, 이를 분석하여 임플란트시술 가능여부를 판단할 수 있게 하는 Software Program이 한층 더 발전할 것이라 사료되어 진다. 이를 치과영역에서 이용한다면, 임플란트시술의 치료효과에 정확성을 높여 줄 뿐만 아니라, 시술을 진행하는 치과의사에게 3차원적인 치근의 구조와 시술부위에 해당하는 해면골의 골밀도 DATA를 동시에 제공할 수 있는 양질의 검사로 자리잡을 수 있을 것으로 기대된다.

영문 초록

I. Purpose To find Sensitivity of Mandible QCT and significance in two tests by comparing T-Score value between Mandible QCT and DEXA. II. Meterial and Methods From March 2, 2010 To May 4, 2010, the Target is total 20 patients performed QCT and DEXA scan at the same time. The average age is 70.7 years old, the men is 5 patients, 15 patients were women. To compare the significance Between QCT and DEXA, Each study were compared and analized by measuring the average T-Score values. Extended Brilliance Workspace (Version 3.5, Philips Medical System, Netherlands) was used as analize devices of QCT T-Score and The Lunar Dual Energy X-Ray Bone Densitometa (Prodigy Advance, GE Health care) was used as analize device of DEXA T-Score. SPSS (Version 12.0 For Window) was used as Statistical tool. III. Result The results by ANOVA analysis between average T-Score value in each group (Mandible QCT and L-Spine DEXA, Total DEXA) showed a slight but almost same differences (P-value 0.001). IV. Conclusions If Bone Mineral Density DATA of QCT applied to Mandible can be built to DB (Data base), Software Program to determine whether implant is possible will be developed getting more. If we use Software Program In the area of Dentist, this study is exoected to high quality test that It will improve the accuracy of the therapeutic effect on the implant as well as, it will be able to offer 3D tooth roots structures and BMD DATA corresponding to operating regions to The dentist proceeding implant at the sametime.
