샤임플러그 사진기와 빛간섭단층촬영계의 새로운 모듈을 이용한 전안부 계측치 비교

국문 초록

목적: 샤임플러그 사진기(Pentacam R [Oculus Inc., Wetzlar, Germany])와 빛간섭단층촬영계(Cirrus HD-OCT 5000 R [Carl Zeiss Meditec, Dublin, CA, USA])의 새로운 전안부 모듈을 이용하여 전방 계측치를 비교해 보고자 하였다. 대상과 방법: 백내장수술을 목적으로 내원한 환자 26명, 47안을 대상으로 전방 계측치를 측정하였다. Pentacam R 과 Cirrus HD-OCT 5000 R 의 새로운 전안부 모듈을 이용하여 중심각막두께, 전방 깊이, 전방각(비측, 이측), 전방각 사이 거리를 측정하여 비교하였다. 결과: Pentacam R 과 Cirrus HD-OCT 5000 R 의 새로운 전안부 모듈로 측정한 중심각막두께는 각각 561.68 ± 31.35 μm, 559.04 ± 36.43 μm로 측정되었다. 두 결과값에서는 통계적으로 유의한 차이를 보이지 않았고( p =0.387), 높은 상관관계를 보였다(r=0.824, p <0.001). 전방 깊이는 각각 2.70 ± 0.44 mm, 2.68 ± 0.42 mm로 기기 간 차이를 보이지 않았고( p =0.091), 매우 높은 상관관계를 보였다(r=0.981, p <0.001). 두 가지 방법으로 측정한 전방각(비측, 이측)과 전방각 사이 거리는 모두 유의한 차이를 보이지 않았고(각 각 p =0.109, p =0.153, p =0.322), 모든 그룹에서 높은 상관관계를 보였다(각각 r=0.944, r=0.951, r=0.955, p <0.001 in all groups). Bland Altman plot으로 분석한 일치도는 중심각막두께만을 제외하고 모두 높게 측정되었다. 결론: 노년층 환자에서 샤임플러그 사진기와 빛간섭단층촬영계의 새로운 전안부 모듈로 측정한 중심각막두께, 전방 깊이, 전방각, 전 방각 사이 거리는 유의한 차이를 보이지 않았다. 하지만 중심각막두께는 낮은 일치도를 보여 기기 간 상호 대치가 어려울 수 있으나, 전방 깊이, 전방각, 전방각 사이의 거리에서는 높은 일치도를 보여 임상적으로 대치하여 사용할 수 있을 것으로 생각된다.

영문 초록

Purpose: We compared the anterior segment measurements using a Scheimpflug camera (Pentacam R [Oculus Inc., Wetzlar, Germany]) and a new anterior segment module of optical coherence tomography (Cirrus HD-OCT 5000 R [Carl Zeiss Meditec, Dublin, CA, USA]). Methods: Anterior segment measurements were evaluated in 47 eyes of 26 patients who visited for the purpose of cataract surgery. Measurements of the central corneal thickness (CCT), anterior chamber depth (ACD), anterior chamber angle (ACA; nasal and temporal side), and angle to angle distance (ATA) were compared between the Pentacam R and the new anterior seg-ment module of the Cirrus HD-OCT 5000 R . Results: The mean CCTs measured by the Pentacam R and the new anterior segment module of the Cirrus HD-OCT 5000 R were 561.68 ± 31.35 μm and 559.04 ± 36.43 μm, respectively. There was no statistically significant difference (p = 0.387) and high cor-relations in the results using the two methods (r = 0.824, p < 0.001). The mean ACD measurements using the Pentacam R and Cirrus HD-OCT 5000 R were 2.70 ± 0.44 mm and 2.68 ± 0.42 mm, respectively. The measurements were strongly correlated (r = 0.981, p < 0.001) without a statistically significant difference (p = 0.091). The measurements of ACA (nasal and temporal side) and ATA did not differ significantly different between the two methods (p = 0.109, p = 0.153, p = 0.322, respectively). High corre-lations were observed between the two methods (r = 0.944, r = 0.951, r = 0.955, respectively; p < 0.001 in all groups). Conclusions: There was no significant difference between the CCT, ACD, ACA, and ATA measured by the Pentacam R and the new anterior segment module of the Cirrus HD-OCT 5000 R in adult patients. However, there was a low degree of agreement for the CCT. The measurements of ACD, ACA, and ATA can therefore be considered interchangeable when used clinically.
