외상성 뇌손상 환자의 손 기능회복에 인지운동치료의 효과

국문 초록

목적: 외상성 뇌손상 환자에게 감각인식 훈련과 손바닥 조직화 훈련이 상지기능 및 일상생활활동에 미치는 영향과 그에 따른 변화를 알아보고자 하였다. 방법: 본 연구는 외상성 뇌손상으로 우측 편마비를 가진 24세 남자를 대상으로 2016년 9월 01일부터 9월 30일까지 4주동안 주 5회, 총19회로 1회 30분씩 인지운동치료를 적용하여 손목과 손가락의 감각인식 훈련과 손바닥 조직화 훈련을 실시하였다. 손목관절과 손가락 관절에 관절각 훈련과 손바닥에서 거리인식 훈련을 실시하였으며, 객관적인 평가로 상지기능 평가는 뇌졸중 상지기능검사(MFT), 일상생활활동은 일상생활 자립도 평가 도구(MBI)를 사용하였고, 주관적 평가로는 인지운동치료 프로파일을 사용하였다. 결과: 본 연구에서 대상자는 초기평가에 비해 최종평가에서 상지기능이 2점 향상 되었고 일상생활동작 평가에서는 3점 향상 되었다. 인지운동치료 프로 파일에서는 병리적 특성중 비정상적인 방산 반응과 신장 반사가 억제되는 결과가 나타났고, 감각인식능력과 언어적 표현 능력이 구체화 되는 모습을 보였다. 결론: 본 연구에서 외상성 뇌손상 환자에게 상지의 감각인식 훈련과 손바닥 조직화 훈련이 상지기능과 일상생활활동에 긍정적인 영향을 주었고, 감각인식의 개선에 효과가 있었다.

영문 초록

Objective : This study attempts to look into the influence that sensory recognition training and palm organization training have on the upper limb function and the activities of daily living (ADL) for traumatic brain injury patients. It also examines the changes that come from that influence. Methods : The subject of this study is a 24 year old man who has a right hemiplegia due to traumatic brain injury. From September 1st. 2016 to September 30th, 2016, for a total of 19 times (five times a week for 4 weeks) with each time lasting 30 minutes, cognitive therapeutic exercise was applied and sensory recognition training for the hand and fingers and palm organization training was carried out. For the wrist joint and linger joint, joint recognition training, and for the palm, distance recognition, were conducted. For objective evaluation, MFT was used for upper limb function and MBI for the activities of daily living (ADL). For subjective evaluation, CTE profile was used. Results : In this study, in comparison to the initial evaluation, the subject’s upper limb function was enhanced by 2 points and his activities of daily living by 3 points in the final evaluation. For the cognitive therapeutic exercise profile, results showed a restraint in pathological characteristics, and sensory recognition and linguistic expressions were specified. Conclusion : For the traumatic brain injury patient, the sensory recognition training of the upper limb and the organization training of the palm influenced the upper limb function and activities of daily living positively, and was effective in improving sensory recognition.
