국문 초록

본 연구는 65세 이상 노인을 대상으로 실시한 2014년 노인실태조사의 전국 조사 자료를 활용하여 한국 노인의 저작능력이 인지기능에 미치는 영향을 파악하고 신체기능과 우울의 매개효과를 확인하기 위한 이차자료분석연구이다. 본 연구는 2014년 노인실태조사자료의 응답자 총 10,451명을 대상으로 하였으며 자료수집기간은 2014년 6월 11일~2014년 9월 4일까지였다. 본 연구에서 저작능력은 일상생활 불편감 중 씹기(고기나 딱딱한 것) 능력에 관한 도구를 이용하였으며, 인지기능은 MMSE-DS, 신체기능은 K-IADL, 우울은 SGDS-K 도구를 이용하여 측정하였다. 본 연구에서 수집된 자료는 SPSS 24.0프로그램과 Process Macro를 이용하여 분석하였고, 저작능력이 인지기능에 미치는 영향에 대하여 신체기능과 우울의 매개효과와 이중매개효과 검정은 Hayes(2013)의 Bootstrapping 방법에 기반한 Process Macro를 이용하여 분석하였다. 본 연구결과, 첫째, 대상자의 저작능력이 인지기능에 미치는 총 효과(p>.05)와 직접효과(p>.05)는 유의하지 않은 것으로 나타났다. 둘 째, 대상자의 저작능력은 신체기능을 단일 매개하여 인지기능에 미치는 완전매개 효과(p<.05)가 있는 것으로 나타났다. 셋째, 대상자의 저작능력은 우울을 단일 매개하여 인지기능에 미치는 완전매개 효과(p<.05)가 있는것으로 나타났다. 넷째, 대상자의 저작능력이 신체기능과 우울을 이중 매개하여 인지기능에 미치는 완전매개효과(p<.05)가 있는 것으로 나타났다. 이롤 통해 본 연구는 한국노인의 인지기능 저하를 예방하기 위해서는 저작능력을 강화하고, 신체기능을 독립적으로 유지하고 우울을 예방하기 위한 프로그램 개발이 필요하고, 포괄적 노인건강사정 시 노인의 인지기능 저하에 영향을 주는 직, 간접적 요인을 주기적으로 모니터링 하는 체계적 전략이 필요함을 시사해 주었다.

영문 초록

The purpose of this study was to identify the impact of chewing ability on cognitive function of older people in Korea and the mediation effect of physical function and depression using the nationwide data from “Survey of the Living Conditions and Welfare Needs of Korean Older Persons in 2014”, in Korea. This study was conducted on a total of 10,451 respondents in the “Survey of the Living Conditions and Welfare Needs of Korean Older Persons in 2014” and the data collection was from June 11, to September 4, 2014. Chewing ability was used in the scale on level of daily life inconvenience in chewing ability (meat or hardness). Cognitive function was measured using MMSE-DS, physical function using K-IADL, and depression using SGDS-K. The data were analyzed using the SPSS 24.0 program and Process Macro. Total effects of chewing ability on cognitive function were not significant(p>.05). Direct effects of chewing ability on cognitive function were not significant(p>.05). There were the dual mediation effect(p<.05) of physical function and depression, a complete mediating effect by physical function(p<.05) or depression(p<.05) in the impact of chewing ability on cognitive function of older people in Korea. These results indicate that physical function and depression effect directly and indirectly in the impact of chewing ability on cognitive function of older people. In conclusion, it is suggested that to prevent the decline of cognitive function in older people, it is necessary to consider not only nursing strategies for strengthening chewing ability and physical function, but also nursing interventions for relieving or preventing depression in older people.
