영문 초록

Objectives ： This study aimed to investigate the factors contributing to differences between alcohol dependence patients who do and who do not experience alcohol withdrawal symp-toms. Furthermore, this study investigated factors that may be pertinent to predicting the degree and duration of alcohol withdrawal symptoms. Methods ： Participants were 45 hospital-ized male patients who met the DSM-IV criteria for alcohol dependence. To assess their degree of alcohol withdrawal symptoms, we administered the revised Clinical Institute Withdrawal Assessment for Alcohol (CIWA-Ar). Then, the participants were divided into two groups--one with with-drawal symptoms and the other without--according to their CIWA-Ar scores. We investigated each patient’s sociodemo-graphic factors, alcohol-related factors, clinical laboratory tests, psychopathology, and heart rate variability (HRV), com-paring the two groups’ data to identify their differences. In addition, we carried out a multiple regression analysis to identify factors affecting CIWA-Ar scores and withdrawal symptom duration. Results ： Of the patients with withdrawal symptoms, 21 had CIWA-Ar scores of 10 or higher, while 24 patients had CIWA-Ar scores lower than 10. The two groups showed significant differences in aspartate aminotransferase level (p=0.024), Hamilton rating scale for depression score (p=0.001), and du-ration of their alcohol dependence (p=0.019). The multiple regression analysis showed that aspartate aminotransferase level (p<0.001), pulse rate (p=0.001), and Hamilton rating scale for depression score (p=0.001) affected highest CIWA-Ar scores and that aspartate aminotransferase (p<0.001), pulse rate (p=0.001), Hamilton rating scale for depression score (p=0.004), and HRV index (p=0.011) affected withdrawal symptom duration. Conclusion : This study identified several factors in alcohol dependence patients that may cause the differences between the patients who experience withdrawal symptoms and those who do not experience withdrawal symptoms. Further-more, we identified some factors that affect the degree and duration of withdrawal symptoms. These findings suggest different approaches may be needed for the treatment of alcohol dependence patients based on the degree and duration of their alcohol withdrawal symptoms.
