악관절액 결핍이 악관절 조직에 미치는 영향

국문 초록

저자는 악관절액의 결핍이 악관절 조직에 미치는 영향을 알기 위하여, 가토를 전신마취시킨 후 악관절 부위를 절개하여 상관절강을 노출시킨 다음, 관절강을 생리식염수로 세척한 후 rubber drain 을 삽입하여 관절액의 지속적 결핍을 유도하였다. 가토 총 15마리를 사용하여 대조군과 실험군으로 나누어, 술후 1, 2, 3, 4주에 3마리씩 희생시켜 hematoxylin & eosin으로 염색한 후, 광학현미경으로 관찰하여 다음과 같은 결과를 얻었다. 1. 정상군의 활막내막의 층수는 2∼3층이었으며, 실험군 1주에서는 미만성 세포증식에 의하여 8∼10층으로 되었으나, 점차 감소하여 4주군에서는 4∼5층으로 되었다. 세포간 경계는 불분명하였고, 세포배열은 불규칙하였으며 세포핵은 농염성을 보였다. 2. 염증, 혈관변화 및 내막하조직의 부종은 실험군 2주에서 가장 심하였으며, 실험 4주까지 지속되었다. 3. 내막하조직의 섬유화는 실험군 3주에서 발생하여, 실험 4주에서는 더욱 증가되었다. 4. 관절강의 섬유화에 의하여 실험군 3주에서 관절강이 좁아진 것을 볼 수 있었으며, 실험 4주에서는 더욱 증가되었고, 하관절강에는 유착 소견도 나타났다. 5. 관절원판은 경도의 점액성 변화와 함께 교원섬유의 변성이 실험 2주에서 발생하여, 실험 4주에는 변성이 증가되었다.

영문 초록

The synovial membrane of the temporomandibular joint lines all the intraarticular structures except the articular cartilage of the articular eminence, fossa, the mandicular condyle and the articular disc. The synovial membrane consists of two layers. The first layer is a lining cell layer facing the joint space and called synovial intima. The second layer is called subintimal or subsynovial tissue. The synovial membrane fulfills several important functions in maintaining normal joint physiology and joint function. Synovitis occurs when the level of cellular debris and the concentration of chemical mediators of inflammation produce levels that the synovial membrane is unable to ingest, absorb or process. So degeneration of temporomandibular joint may be increased by dificiency of synovial fluid. After general anesthesia by intramuscular infection of ketamin, exposure of TMJ of rabbit was done by preauricular approach. After exposure, rubber drain was inserted into the upper joint cavity, to prevent accumulation of synovial fluid. Penicillin G was used to prevent secondary infection for 3 days. Fifteen Rabbits were used for experiment, and subdivided into 5 group (one normal group and 4 experimental groups, each subdivided group consisted in 3 rabbits). Experimental group were taken after period of 1, 2, 3, 4 weeks. After euthansia, on bloc excision of TMJ was done to make biopsy specimen. The results were as follows ; 1. Layers of synovial intima were increased in experimental group, 8∼10 layers in first week group and 4∼5 layers in fourth week group as compared with 2∼3 layers in normal group. 2. Inflammation, vascular change and subintimal edema were highest in second week group, but continued to fourth week group. 3. Subintimal fibrosis occured in second week group, and increased to fourth week group. 4. Due to fibrosis of joint cavity space occured in the third week group, and increased to fourth week group. 5. Degeneration of articular disc occurred in second week group and increased to fourth week group.
