국문 초록

전산화단층촬영(CT) 조영 검사 시 18∼20G 바늘이 사용되고, 검사 후 환자는 일반적으로 알코올솜을 이용한 자 가 압박 지혈(Conventional Method: CM)을 하며 충분하지 못한 압박으로 종종 출혈을 목격한다. 이에 출혈을 방 지하기 위하여 일회용 투명 지혈대(Transparent Tourniquet: TT)를 개발했으며, 기존 지혈 방법과 만족도를 비교하 여 유용성을 증명하고자 하였다. CT 경험이 있는 환자 50명, CT 근무 중인 간호사 25명으로 조사는 설문지 개발을 통한 개별 인터뷰 방식이었 고, 5점 척도를 이용하여 기존 CM과 TT 방식의 전체적 만족도의 평균을 대응 T-test를 하였다. 또한 TT에 대한 환자 대상 설문은 편리, 안전. 위생, 착용감을 연령과 성별로, 간호사의 경우는 기능, 편리, 안전, 위생, 지혈정도를 간호 경력에 따른 만족도 차이를 교차분석을 실시하였다. 환자 설문 결과는 전체적 만족도는 TT가 4.34±0.51로 CM이 2.5±0.81 보다 높았으며 p<0.05로 통계적으로도 유의하였으며, 편리, 안전. 위생, 착용감이 각각 4.40±0.53, 4.28±0.57, 4.52±0.54, 4.10±0.54이며 연령과 성별에 따른 차이는 크게 없이 평균이 높게 나왔다. 간호사 설문 결 과는 전체적 만족도는 TT가 3.8±0.33로 CM이 3.5±0.96 보다 높았으나 p>0.05로 통계적으로는 유의하지 않았다. 기능, 편리, 안전, 위생, 지혈정도는 각각 3.52±0.77, 3.84±0.68, 3.64±0.48, 3.40±0.50, 3.92±0.49이며 간호경력 에 따른 큰 차이는 없었다. 새로 개발된 지혈대는 환자와 사용자 모두 기존 알코올 솜 보다 높은 만족도를 나타내 고, 각 평가 항목 또한 높은 평균을 나타내 유용성이 증명되었으며, 환자 위생과 안전에 크게 도움될 것으로 사료 된다.

영문 초록

The 18~20G needle is used to computer tomography (CT) contrast examination. Therefore, a patient has to apply a self-administering hemostasis (conventional method: CM) and often experience bleeding in the course. Thus, we developed the new disposable transparent tourniquet (TT) for reducing. This study was to compare the usefulness between the proposed transparent tourniquet and the existing hemostatic methods. A Satisfaction survey was conducted by 50 patients and 25nurses. The survey contained the satisfaction of the convenience, safety, sanitation, and wearing sensation of transparent tourniquet. We employed face-to-face interview on 5 points likert scales. And Chi-square, paired T-test were used for the statistics verification. As for the patients, the satisfaction levels were measured for each category with the gender, age. Patients evaluation, overall satisfaction high average sore used TT and there were statistical significance by paired T-test(p<0.05). The following is the average satisfaction level for each category: 4.4±0.53 in; 4.28±0.57 in safety; 4.52±0.54 in sanitation; 4.16±0.54 in wearing sensation. So the overall satisfaction level is measured at 4.34±0.51. As for the nurses, CT work experience and the current satisfaction with tourniquet were counted as variables. The satisfaction level for each category is: 3.8±0.7 in; 3.6±0.68 in safety; 3.4±0.5 in sanitation; 3.9±0.49 in hemostasis. The overall satisfaction level is 3.8±0.3. Patients’ satisfaction levels were very high with little difference among variables. Nurses’ satisfaction levels were different with the TT depending on their work experience but their overall satisfaction was high. This TT will be a starting point to minimizing patient s inconvenience and more studies are necessary to enhance their satisfaction.
