우안 3번, 4번, 6번 뇌신경 마비를 동반한 두개강내 국균증 1예

국문 초록

목적: 우안 3번, 4번, 6번 뇌신경 마비를 동반한 두개강내 국균증 1예를 경험하였기에 이를 보고하고자 한다. 증례요약: 66세 여자 환자가 5일 전부터 시작된 우안의 경한 안통, 복시, 눈꺼풀처짐, 안구운동장애를 주소로 내원하였다. 환자는 내원3주 전부터 우측 관자놀이 주변의 통증과 콧물을 주소로 이비인후과에서 우측 접형동의 부비동염으로 경구 항생제를 7일간 복용하였던 과거력이 있었다. 눈꺼풀 각막반사간거리는 우안 -4 mm, 좌안 +4 mm이었고, 우안 상전 -4, 하전 -4, 내전 -3.5, 외전 -2.5의 안구운동장애가 관찰되었다. 부비동 자기공명영상에서 우측 비인두에서 시작하여 추체 첨부, 파열공, 해면정맥동, 접형동을 거쳐 안와첨 및 하안와열을 침범하는 T2에서 증가한 신호강도를 보이는 종괴가 관찰되었다. 안과 내원 5일 후 이비인후과에서 내시경적 부비동수술을 시행하여 종괴를 제거하였고 종괴의 병리조직검사에서 국균을 확인하였다. 환자는 수술 후 11일 동안 Voriconazole 정맥주사및 11주 동안 경구 Voriconazole을 복용하였다. 수술 6주 후부터 눈꺼풀처짐 및 안구운동장애는 호전을 보였으며 수술 4개월 후에는안구운동장애가 완전히 없어졌다. 그 이후 1년간의 경과 관찰 동안 재발 소견은 보이지 않고 있다. 결론: 안와국균증 환자에서 두개강내를 침범하여 안구돌출 없이도 3번, 4번, 6번 뇌신경 마비가 동시에 발생할 수 있으며, 안구운동장애, 눈꺼풀처짐, 부비동염의 증상이 있을 때에는 자기공명영상검사를 비롯한 진단 도구의 적절한 이용과 적극적인 치료가 필요할 것으로 생각한다.

영문 초록

Purpose: To report a case of cerebral aspergillosis with third, fourth, and sixth nerve palsy. Case summary: A 66-year-old female presented with ocular pain, diplopia, ptosis, and limited ocular movement of the right eye.The patient had experienced rhinorrhea and headache in the right temporal area 3 weeks prior and was treated with oral antibiotics for 1 week. Marginal reflex distance 1 was -4 mm in the right eye and +4 mm in the left eye. Upward, downward, medial,and lateral gaze limitation (-4/-4/-3.5/-2.5) was evaluated. Magnetic resonance imaging (MRI) revealed a mass originating fromthe nasopharynx and passing through the petrous apex, foramen lacerum, carvernous sinus, sphenoid sinus, orbital apex, and inferior orbital fissure. The mass had high signals on T2-weighted imaging. After 5 days, the mass was removed by endoscopic surgery and aspergillus was detected histopathologically. The patient was given intravenous voriconazole for 11 days and oral voriconazole for 11 weeks. Ptosis and ocular movement limitation began to improve after 6 weeks postoperatively. After 4 months, ocular movement was not limited and there was no recurrence during the 1 year follow-up period. Conclusions: The present case showed that orbital aspergillosis can invade the intracranial area and third, fourth, and sixth nerve palsy can develop without exophthalmos. Thus, when ocular movement disorders, ptosis and symptoms of sinusitis are present in orbital aspergillosis patients, use of appropriate diagnostic tools such as MRI and active treatment are important.
