재발외사시에서 한 눈 내직근절제술의 용량효과관계

국문 초록

목적: 외사시로 두 눈 외직근후전술을 받은 후 재발한 외사시 환자에서 한 눈 내직근절제술의 용량-효과관계를 알아보았다. 대상과 방법: 외사시로 두 눈 외직근후전술을 받은 후 25 PD 이하의 외사시가 재발한 환자 중 한눈 내직근절제술을 시행한 39명을 대상으로 의무기록을 후향적으로 조사하였다. 내직근 절제는 원거리 사시각을 기준으로 최대 7.0 mm까지 0.5 mm 단위로 시행하였고, 술 후 1주일, 3개월, 6개월에 각각 사시각을 측정하여 내직근 절제량에 따른 변화를 조사하였다. 결과: 이차수술 전 외사시각은 21.33 ± 2.96 PD였으며, 평균 6.26 ± 0.57 mm의 내직근을 절제하였다. 수술 후 외사시각은 1주일에0.82 ± 3.47 PD, 3개월에 1.87 ± 3.10 PD, 6개월에 5.40 ± 4.79 PD이었다. 수술 후 1주일, 3개월, 6개월에서 mm당 각각의 교정효과는 3.29 ± 0.68, 3.17 ± 0.58, 2.75 ± 0.89 PD이었다. 절제량에 따른 mm당 교정효과는 유의한 차이는 없었다(p=0.423, 0.382).수술 성공률은 1주일에서 89.75%, 3개월에서 92.31%, 6개월에서 87.18%이었다. 예측한 절제량에 따른 mm당 교정효과는 3개월에서3.04-3.22 PD/mm, 6개월에서 2.79-2.82 PD/mm로 비교적 일정하게 유지되었다. 결론: 두 눈 외직근후전술 후 재발한 외사시에서 한 눈 내직근절제술 시 mm당 교정효과는 시간이 경과함에 따라 감소하였으며, 수술량에 따른 mm당 교정효과는 큰 차이가 없었다. 산출한 용량효과관계가 유용한 지표가 될 것으로 생각한다.

영문 초록

Purpose: To investigate the dose-effect relationship of unilateral medial rectus resection for recurrent exotropia after bilateral lateral rectus recession. Methods: This study comprised 39 patients who underwent unilateral medial rectus resection for recurrent exotropia under 25PD and bilateral rectus recession for prior surgery of exotropia. The medial rectus was resected from 5.0 to 7.0 mm according to angle deviation at a distance. The postoperative deviated angle was checked at one week, 3 months and 6 months postoperatively to investigate the amount of corrected deviation per resected muscle. Results: The average preoperative deviation was 21.33 ± 2.96 PD. We resected mean 6.26 ± 0.57 mm of the medial rectus muscle. The postoperative deviated angle was 0.82 ± 3.47, 1.87 ± 3.10 and 5.40 ± 4.79 PD at 1 week, 3 months and 6 months after surgery, respectively. The corrected deviated angle per millimeter (mm) was 3.29 ± 0.68, 3.17 ± 0.58 and 2.75 ± 0.89 PD at 1 week, 3 months and 6 months after surgery. There were no significant differences among the changes of postoperative deviation per millimeter according to the amount of medial rectus (MR) resection (p-value = 0.423, 0.382) The success rate was 89.75%, 92.31% and 87.18% at 1 week, 3 months and 6 months after surgery, respectively. The expected corrected angle according to the amount of resection remained constant and was 3.04-3.22 PD/mm at 3 months after surgery and 2.79-2.82PD/mm at 6 months after surgery. Conclusions: The dose-effect per millimeter was decreased as time passed. There was no statistical difference among the corrected deviation angles per millimeter according to the amount of MR resection. We expect that the calculated dose-effect relationship may be a useful guideline for unilateral medial rectus resection for recurrent exotropia.
